Medicare's push to improve chronic care attracts businesses, but not many doctors

easy trip planners news today

Medicare enrollees with two or more chronic conditions are eligible for Chronic Care Management, which pays doctors to check in with those patients monthly. But the service hasn't caught on. John Moore/Getty Images hide caption

Medicare enrollees with two or more chronic conditions are eligible for Chronic Care Management, which pays doctors to check in with those patients monthly. But the service hasn't caught on.

Carrie Lester looks forward to the phone call every Thursday from her doctors' medical assistant, who asks how she's doing and if she needs prescription refills. The assistant counsels her on dealing with anxiety and her other health issues.

Lester credits the chats for keeping her out of the hospital and reducing the need for clinic visits to manage chronic conditions including depression, fibromyalgia and hypertension.

"Just knowing someone is going to check on me is comforting," says Lester, 73, who lives with her dogs, Sophie and Dolly, in Independence, Kansas.

At least two-thirds of Medicare enrollees have two or more chronic health conditions, federal data shows . That makes them eligible for a federal program that, since 2015, has rewarded doctors for doing more to manage their health outside office visits.

But while early research found the service, called Chronic Care Management, reduced emergency room and in-patient hospital visits and lowered total health spending, uptake has been sluggish.

Federal data from 2019 shows just 4% of potentially eligible enrollees participated in the program, a figure that appears to have held steady through 2023, according to a Mathematica analysis. About 12,000 physicians billed Medicare under the CCM mantle in 2021, according to the latest Medicare data analyzed by KFF Health News. (The Medicare data includes doctors who have annually billed CCM at least a dozen times.)

By comparison, federal data shows about 1 million providers participate in Medicare.

$62 per patient, per month

Even as the strategy has largely failed to live up to its potential, thousands of physicians have boosted their annual pay by participating, and auxiliary for-profit businesses have sprung up to help doctors take advantage of the program. The federal data showed about 4,500 physicians received at least $100,000 each in CCM pay in 2021.

Through the CCM program, Medicare pays to develop a patient care plan, coordinate treatment with specialists, and regularly check in with beneficiaries. Medicare pays doctors a monthly average of $62 per patient , for 20 minutes of work with each, according to companies in the business.

Without the program, providers often have little incentive to spend time coordinating care because they can't bill Medicare for such services.

Health policy experts say a host of factors limit participation in the program. Chief among them is that it requires both doctors and patients to opt in. Doctors may not have the capacity to regularly monitor patients outside office visits. Some also worry about meeting the strict Medicare documentation requirements for reimbursement and are reluctant to ask patients to join a program that may require a monthly copayment if they don't have a supplemental policy.

"This program had potential to have a big impact," says Kenneth Thorpe , an Emory University health policy expert on chronic diseases. "But I knew it was never going to work from the start because it was put together wrong."

He said most doctor's offices are not set up for monitoring patients at home. "This is very time-intensive and not something physicians are used to doing or have time to do," Thorpe says.

For patients, the CCM program is intended to expand the type of care offered in traditional, fee-for-service Medicare to match benefits that — at least in theory — they may get through Medicare Advantage, which is administered by private insurers.

But the CCM program is open to both Medicare and Medicare Advantage beneficiaries.

The program was also intended to boost pay to primary care doctors and other physicians who are paid significantly less by Medicare than specialists, says Mark Miller, a former executive director of the Medicare Payment Advisory Commission, which advises Congress. He's currently an executive vice president of Arnold Ventures, a philanthropic organization focused on health policy. (The organization has also provided funding for KFF Health News .)

Not "easy money"

Despite the allure of extra money, some physicians have been put off by the program's upfront costs.

"It may seem like easy money for a physician practice, but it is not," says Dr. Namirah Jamshed, a physician at UT Southwestern Medical Center in Dallas.

Jamshed says the CCM program was cumbersome to implement because her practice was not used to documenting time spent with patients outside the office, a challenge that included finding a way to integrate the data into electronic health records. Another challenge was hiring staff to handle patient calls before her practice started getting reimbursed by the program.

Only about 10% of the practice's Medicare patients are enrolled in CCM, she says.

Jamshed says her practice has been approached by private companies looking to do the work, but the practice demurred out of concerns about sharing patients' health information and the cost of retaining the companies. Those companies can take more than half of what Medicare pays doctors for their CCM work.

Dr. Jennifer Bacani McKenney, who runs a family medicine practice in Fredonia, Kansas, with her father — where Carrie Lester is a patient — says the CCM program has worked well.

She says having a system to keep in touch with patients at least once a month has reduced their use of emergency rooms — including for some who were prone to visits for nonemergency reasons, such as running out of medication or even feeling lonely. The CCM funding enables the practice's medical assistant to call patients regularly to check in, something it could not afford before.

For a small practice, having a staffer who can generate extra revenue makes a big difference, McKenney says.

While she estimates about 90% of their patients would qualify for the program, only about 20% are enrolled. One reason is that not everyone needs or wants the calls, she says.

While the program has captured interest among internists and family medicine doctors, it has also paid out hundreds of thousands of dollars to specialists, such as those in cardiology, urology and gastroenterology, the KFF Health News analysis finds. Primary care doctors are often seen as the ones who coordinate patient care, making the payments to specialists notable.

A federally funded study by Mathematica in 2017 found the CCM program saves Medicare $74 per patient per month, or $888 per patient per year — due mostly to a decreased need for hospital care.

The study quoted providers who were unhappy with attempts to outsource CCM work. "Third-party companies out there turn this into a racket," the study cites one physician saying, noting that companies employ nurses who don't know patients.

Nancy McCall, a Mathematica researcher who co-authored the 2017 study, says doctors are not the only resistance point. "Patients may not want to be bothered or asked if they are exercising or losing weight or watching their salt intake," she says.

How outsourcing works

Still, some physician groups say it's convenient to outsource the program.

UnityPoint Health, a large integrated health system based in Iowa, tried doing chronic care management on its own, but found it administratively burdensome, says Dawn Welling, the UnityPoint Clinic's chief nursing officer.

For the past year, it has contracted with a Miami-based company, HealthSnap, to enroll patients, have its nurses make check-in calls each month, and help with billing. HealthSnap helps manage care for more than 16,000 of UnityPoint Health's Medicare patients — a small fraction of its Medicare patients, which includes those enrolled in Medicare Advantage.

Some doctors were anxious about sharing patient records and viewed the program as a sign they weren't doing enough for patients, Welling says. But she says the program has been helpful, particularly to many enrollees who are isolated and need help changing their diet and other behaviors to improve health.

"These are patients who call the clinic regularly and have needs, but not always clinical needs," Welling says.

Samson Magid, CEO of HealthSnap, says more doctors have started participating in CCM since Medicare increased pay in 2022 for 20 minutes of work, to $62 from $41, and added billing codes for additional time.

To help ensure patients pick up the phone, caller ID shows HealthSnap calls as coming from their doctor's office, not from wherever the company's nurse might be located. The company also hires nurses from different regions so they may speak with dialects similar to those of the patients they work with, Magid says.

He says some enrollees have been in the program for three years and many could stay enrolled for life, which means they can bill patients and Medicare long-term.

Blinken: G7 is 'committed to de-escalating'

Antony Blinken , the US secretary of state, addressed the press today at the conclusion of the G7 meeting of foreign ministers in Capri , Italy .

On behalf of the G7, he condemned Iran ’s cruise missile and drone attack on Israel that prompted an airstrike today on Isfahan , that the US says it has confirmed was carried out by Israel . It hasn not yet been confirmed by Israel or Iran.

“We’re committed to Israel’s security,” Blinken said. “We’re also committed to de-escalating.”

He noted that despite the tensions with Israel and Iran , “we remain intensely focused on Gaza”.

This day so far

It is currently 7pm in Gaza , 8pm in Tel Aviv, and 8.30pm in Tehran.

Here is a summary of what has happened so far:

the Iranian foreign minister, Hossein Amirabdollahian, said that drones reportedly fired by Israel at the Iranian city of Isfahan did not cause “any damage or casualties”, as reported by Iranian media.

The US Congress is pushing forward a $95m national security package that would provide additional aid to Israel following Israel’s reported airstrike in Iran. The bill overcame a massive hurdle with the support of Democrats in the House of Representatives. If approved this weekend, it will be sent to the Senate.

A spokesperson for the Iranian foreign ministry denounced the US’s veto of a Palestinian request to the UN security council, blocking the world body’s recognition of a Palestinian state. Iranian diplomat Nasser Kanani called the veto “irresponsible” and “unconstructive”.

Thank you for reading the latest updates on the ongoing crisis in the Middle East.

Drones reportedly fired by Israel at the city of Isfahan in Iran caused no damage or casualties, said the Iranian foreign minister, Reuters reported.

Iranian Foreign Minister Hossein Amirabdollahian gave remarks on the drone attack while speaking with envoys of Muslim nations in New York.

“The Zionist regime’s media supporters, in a desperate effort, tried to make victory out of their defeat, while the downed mini-drones have not caused any damage or casualties,” Amirabdollahian said, according to Iranian media.

Here is an analysis of what Israel’s latest attack on Iran means for the political survival of the Israeli prime minister, Benjamin Netanyahu , from the Guardian’s Bethan McKernan.

In the aftermath of Iran’s unprecedented salvo of missiles and drones fired directly at Israel at the weekend, Benny Gantz, a centrist member of the Israeli war cabinet, said that the country would respond “in the place, time and manner it chooses”. On Friday morning, that turned out to be explosions in the central Iranian city of Isfahan . Although no Israeli official has claimed responsibility for what seem to have been drone strikes on a military installation , Tehran, which retaliated after an airstrike on its consulate in Damascus, has downplayed the incident. The limited response may have for now staved off the threat of regional war. Unclaimed drone attacks on a military site in Isfahan are unlikely to satisfy supporters or critics of Israel’s longtime leader, Benjamin Netanyahu , or restore the level of deterrence Israel enjoyed in the region before the Hamas attack of 7 October. “Netanyahu, who has been threatening to attack Iran for 20 years, has realised a dream, or at least part thereof. The question is what about us, the people,” defence analyst and columnist for the leftwing Israeli daily Haaretz, Amos Harel, wrote on Friday. Meanwhile, Netanyahu ally and the far-right national security minister, Itamar Ben-Gvir, reacted with just one word – “Feeble!” – in a post on X, formerly Twitter. The Israeli prime minister may have achieved several short-term wins here. The muted Iranian reaction has soothed an anxious Israeli public before the week-long Passover holiday. Israel has also struck while it still has international diplomatic support for such an action, before the sense of urgency wanes and the world’s focus returns to the war in Gaza and its deepening famine …

A spokesperson for the foreign affairs ministry of Iran condemned the US’s veto to block full United Nations membership for Palestine, calling the decision “irresponsible”, AFP reported.

The US vetoed the Palestinian request to the UN security council, blocking the world body’s recognition of a Palestinian state.

The US was the only country to oppose the request on the 15-member security council. Twelve nations were in favor. The UK and Switzerland abstained.

“Washington’s action exposed the fraudulent nature of US foreign policy and its isolated position,” Iranian diplomat Nasser Kanani said.

Kanani added that Washington’s veto was “irresponsible” and “unconstructive”.

The US Congress is pushing forward a $95bn national security package that would provide additional aid to Israel following Israel’s airstrike in Iran.

The bill was moved forward in the House of Representatives following a critical procedural vote, the Associated Press reported. The legislation would also provide humanitarian support to Taiwan and Ukraine.

Aid for the international conflict between Russia and Ukraine has received faltering support from far-right conservatives, threatening the bill’s passage.

With the latest procedural obstacle cleared, the bill could be approved by the House this weekend and then sent to the Senate.

Afternoon summary

It has gone 5pm in Gaza , 6pm in Tel Aviv and 6.30pm in Tehran. Here is a summary of the day’s events so far:

Speaking publicly at the G7 meeting in Capri, the US secretary of state, Antony Blinken, declined to elaborate further on the reported Israeli attack on Iran, except to say his country had not been involved. “I’m not going to speak to that except to say that the United States has not been involved in any offensive operations,” Blinken said.

Blinken said while the Gy was “committed” to Israel’s security, “we’re also committed to de-escalating ”. “What the G7 is focused on, and it’s reflected in our statement and in our conversation, is our work to de-escalate tensions, to de-escalate from any potential conflict,” he said. “You saw Israel on the receiving end of an unprecedented attack, but our focus has been on, of course, making sure that Israel can effectively defend itself, but also de-escalating tensions and avoiding conflict. That remains our focus.”

At a separate news conference moments before Blinken, Italy’s foreign minister, Antonio Tajani, said the US was “informed at the last minute” but did not elaborate.

Air defence systems over several Iranian cities were activated, state media reported , after the country’s official broadcaster said explosions were heard near the city of Isfahan , home to sites associated with Iran’s nuclear program, including its underground Natanz enrichment site.

The International Atomic Energy Agency (IAEA) says there is no damage to Iran’s nuclear sites. In a post on X, the agency said it continues to monitor the situation very closely and calls for extreme restraint from all sides, stressing that “nuclear facilities should never be a target in military conflicts”.

French police arrested a man who had threatened to blow himself up at Iran’s consulate in Paris on Friday afternoon. A police source told Reuters that the man was seen at about 11am (9am GMT) entering the consulate, carrying what appeared to be a grenade and explosive vest. The man exited the consulate and was not actually carrying explosives, the source said.

Four Israeli extremist settlers in the occupied West Bank and East Jerusalem have been today sanctioned by the EU along with two entities over alleged “serious human rights abuses against Palestinians”.

The US also added to its settler-related sanctions today by imposing sanctions on two entities that the US said helped raise tens of thousands of dollars for two violent extremists in the West Bank already targeted with US sanctions.

The Turkish foreign minister, Hakan Fidan, on Friday spoke by phone to his Iranian counterpart, Hossein Amirabdollahian, at Iran’s request to discuss regional developments , a Turkish diplomatic source told Reuters. The source did not provide further details.

German airline group Lufthansa has suspended flights to Israel and Iraq until early Saturday. Lufthansa subsidiary Austrian Airlines also decided as a “precautionary measure” to suspend flights to Jordan’s capital Amman, as well as Erbil and Tel Aviv on Friday “to comprehensively reassess the security situation”. Both Lufthansa and Austrian Airlines had already suspended their flights to and from Tehran up until the end of the month.

Polish national airline LOT canceled flights on Friday to Tel Aviv and Beirut due to the unstable situation in the region, a spokesperson was quoted as saying by state news agency PAP.

Here’s a look at the area targeted last night in what the US has confirmed as an Israeli military operation in Iran:

EU imposes sanctions on four Israeli settlers

Lisa O'Carroll

The individuals on the list are: Neria Ben Pazi , 31. According to the EU’s official journal, the reason for the listing was that Ben Pazi “established four of the most violent outposts in the West Bank in 2019”. He was “one of the main perpetrators of the forced displacement of a Bedouin community of Wadi Seeq near Ramalah. His actions “have been likened to torture”, the official journal said, including an attack on 12 October last year in which Palestinians were “severely beaten, handcuffed and photographed in their underwear” as well as “urinated on”.

Another Israeli, Yinon Levi , 32, is accused of “multiple violent acts” including storming and damaging houses of Palestinian families and setting dogs on farmers.

Also on the list is Meir Mordechai Ettinger , 33, “considered a leading figure of the Hilltop Youth, a radical youth group consisting of members known for violent acts against Palestinians and their villages in the West Bank.

According to the official journal he was involved in a deadly arson attack in 2015, that killed two parents and their 18-month-old baby, for which he was detained without charge that same year.

Another member of the group, Names as Elisha Yered , born in 2001, was reported to engage in settler violence through so-called “price tag attacks” including “physical and psychosocial harassment, beatings, murder and demolition of property, against Palestinians is of a systematic nature”.

The journal says he was “part of a group of armed settlers” involved in an attack last year near Ramallah which led to the death of the 19-year-old Palestinian Qusai Jammal Mi’tan and wounded several other Palestinians”.

One of the two entities named is Lehava , which the EU’s official journal describes as a “radical right-wing Jewist supremacist group” which “uses violence and incites violence against Palestinians, Christians and Messianic Jews”. According to the journal, Lehava “organises violent protests against Jewish Muslim weddings and the LGBQTi community”.

The other entity is named as Hilltop Youth , described by the journal as “a radical group consisting of members known for violent acts against Palestinians and their villages in the West Bank”.

US sanctions on entities that raised funds for West Bank settlers

The US treasury department announced today that the US is imposing sanctions on Mount Hebron Fund and Shlom Asiraich – two entities that the US said helped raise tens of thousands of dollars for two violent extremists in the West Bank already targeted with US sanctions.

Mount Hebron Fund launched an online fundraising campaign that raised $140,000 for settler Yinon Levi after he was sanctioned in February for leading a group of settlers that assaulted Palestinian and Bedouin civilians, burned their fields and destroyed their property, the Treasury said.

Shlom Asiraich raised $31,000 for David Chai Chasdai , who the US said had initiated and led a riot that included setting vehicles and buildings on fire and causing damage to property in Huwara , resulting in the death of a Palestinian civilian.

“Mount Hebron Fund and Shlom Asiraich generated tens of thousands of dollars for extremists responsible for destroying property, assaulting civilians, and violence against Palestinians,” said Deputy Secretary of the Treasury Wally Adeyemo. “Such acts by these organizations undermine the peace, security, and stability of the West Bank. We will continue to use our tools to hold those responsible accountable.”

The US has previously sanctioned five settlers and two unauthorized outposts in the West Bank.

At least 34,012 Palestinians have been killed and 76,833 others have been injured by Israel ’s military offensive since the 7 October, Gaza ’s health ministry said on Friday.

The Hama s-run health ministry does not distinguish between combatants and non-combatants.

Man arrested in Paris after police cordon off Iranian consulate due to reported bomb threat

French police arrested a man who had threatened to blow himself up at Iran’s consulate in Paris , police said, Reuters reported.

A police source had told Reuters the man was seen at about 11 am (0900 GMT) entering the consulate, carrying what appeared to be a grenade and explosive vest.

The man exited the consulate and was not actually carrying explosives, a police source said.

Turkish foreign minister Hakan Fidan on Friday spoke by phone to his Iranian counterpart Hossein Amirabdollahian at Iran ’s request to discuss regional developments, a Turkish diplomatic source told Reuters.

The source did not provide further details.

Blinken says conversations on Rafah at senior levels with Israel continue

Further to the comments by Blinken already reported on the blog, there are some more lines from Reuters on what the US secretary of state said on the topic of Gaza .

At a press conference on Friday after a gathering of G7 foreign ministers on the southern Italian island of Capri , Blinken told reporters “we cannot support a major military operation in Rafah”. He said conversations on Rafah , in the southern Gaza Strip , continue at senior levels with Israel .

Blinken said he believed that “Israel’s objectives can be achieved” without a Rafah offensive. He added: “We are committed to achieving a Palestinian state with necessary guarantees for Israel.”

Qatar expressed regret on Friday over the failure of the UN security council to adopt a draft resolution recognising a Palestinian state through full membership of the UN (see 08:14 BST ), according to Reuters.

The United Arab Emirates said on Friday that granting Palestinians full membership in the UNwould be “an important step to boost peace efforts in the region”, the state-run WAM news agency reported.

German airline group Lufthansa has suspended flights to Israel and Iraq until early Saturday.

Services by airlines of the group to Tel Aviv in Israel and Erbil in Iraqi Kurdistan were suspended until 5am GMT due to the “current situation”, a spokesperson told AFP.

Lufthansa subsidiary Austrian Airlines also decided as a “precautionary measure” to suspend flights to Jordan ’s capital Amman , as well as Erbil and Tel Aviv on Friday “to comprehensively reassess the security situation”.

“Austrian Airlines continuously monitors and assesses the security situation in the Middle East and is in close contact with the authorities,” Austrian Airlines said in a statement sent to AFP.

Both Lufthansa and Austrian Airlines had already suspended their flights to and from Tehran up until the end of the month.

“Today’s flight 151/152 to Israel from Warsaw and to Beirut 143/144 have been cancelled,” Krzysztof Moczulski told PAP, according to a Reuters report. Moczulski said decisions about future flights would be made on an ongoing basis.

US secretary of state Antony Blinken repeatedly declined to confirm a reported Israeli attack on Iran on Friday, saying Washington has not been involved in any offensive operations and it was committed to de-escalating tensions in the region, reports Reuters.

“I’m not going to speak to that except to say that the United States has not been involved in any offensive operations,” Blinken said at a news conference capping a gathering of G7 foreign ministers on the southern Italian island of Capri . According to Reuters reporters, the top US diplomat kept repeating the same response, almost verbatim, when he was asked about the issue several times at the news conference.

US secretary of state Antony Blinken is pictured next to a US flag at a press conference at the end of the G7 foreign ministers meeting on the Italian island of Capri on Friday.

At a separate news conference moments before Blinken, Italy’s foreign minister Antonio Tajani said the US was “informed at the last minute” but did not elaborate (see 11:58 BST ).

Explosions echoed over the Iranian city of Isfahan in the early hours of Friday in what sources described as an Israeli attack, say Reuters, but Tehran played down the incident and indicated it had no plans for retaliation – a response that appeared gauged towards averting region-wide war.

Israel said nothing about the incident. It had said for days it was planning to retaliate against Iran for Saturday’s strikes, the first direct attack on Israel by Iran in decades of shadow war waged by proxies which has escalated throughout the Middle East .

United Nations secretary general António Guterres said on Friday that “it is high time to stop the dangerous cycle of retaliation in the Middle East,” his spokesperson said in a statement.

“The secretary general condemns any act of retaliation and appeals to the international community to work together to prevent any further development that could lead to devastating consequences for the entire region and beyond,” Stéphane Dujarric said, accroding to Reuters.

Iran has been trying to reassure people that the country did not face a major threat after airstrikes were carried out in the Isfahan province on Friday.

A news anchor said everything was “back to normal” in a report aired on Press TV, the Iranian state broadcaster’s English-language outlet.

In another clip, said to be filmed in Isfahan and published by Iranian media, an unidentified person is heard saying there had been no damage to the area.

Officials in Washington said Israeli forces were carrying out military operations against Iran but did not describe the character or scale of those operations. Israel and Iran are yet to confirm the source of the strikes.

We are pausing our live coverage of Russia's invasion of Ukraine today.

We'll be back again with more updates.

Here's a round-up of today, which started with the deaths of at least eight people in a major Russian missile attack in central Ukraine's Dnipropetrovsk region.

Volodymyr Zelenskyy called for more air defences and said: "Russia must be held accountable for its terror, and every missile, every Shahed [drone] must be shot down."

In a speech after a meeting of the Group of Seven (G7) major democracies on the Italian island of Capri, US Secretary of State Antony Blinken said it is "imperative" Ukraine gets more resources immediately to help it fight Russia.

"It needs more air defences, it needs more munitions, it needs more artillery - allies and partners including the G7 countries are committed to delivering on that," he added.

Meanwhile, Ukraine claimed to have shot down a Russian Tu-22M3 bomber "for the first time". Russia's defence ministry said the crash appeared to have been caused by a technical malfunction.

Two detained in Poland after hammer attack on Navalny aide

Two people were detained in Poland on suspicion of a hammer attack on Leonid Volkov,  the exiled top aide of the late Russian opposition leader Alexei Navalny.

Mr Volkov had blamed Vladimir Putin for the attack and he thanked Lithuanian police for working "energetically and persistently" over the past month on his case.

"I am very glad that this work has been effective", he tweeted. "Well, we'll find out the details soon. Can't wait to find out!"

And a Polish man was arrested over allegations of being ready to help Russia's military intelligence in an alleged plot to assassinate Volodymyr Zelenskyy.

Volodymyr Zelenskyy has made an emotional appeal to NATO members, saying the current level of foreign aid for Ukraine is "very limited".

The Ukrainian president said Israel had not been left to fend for itself during Iran's aerial bombardment on Saturday.

During that attack, Western allies had stepped in to help shoot down Iranian drones and missiles.

"Putin must be brought down to earth and our sky must become safe again... And it depends fully on your choice... [the] choice whether we are indeed allies," Mr Zelenskyy said in his speech.

He said Ukraine needs a minimum of seven Patriot or other high-end air defence systems to counter Russian air strikes.

Russia has recently intensified its bombardment of Ukraine, and the UN reported civilian deaths had sharply increased in March.

NATO ministers have decided to supply more air defence systems to Ukraine, the organisation's secretary general has said.

Jens Stoltenberg said several NATO allies had made concrete commitments which he expected to be announced soon.

"In addition to Patriots, there are other weapons that allies can provide, including [French system] SAMP/T and many others, who do not have available systems, have pledged to provide financial support to purchase them for Ukraine," he told reporters in Brussels.

A Russian missile attack has damaged port infrastructure in Ukraine's southern Odessa region, the governor has said.

One person was injured, he added.

This map by the British Ministry of Defence shows the latest update on advances in Russia's invasion of Ukraine.

The head of the European Union's executive branch has visited Finland's border with Russia to assess the security situation there.

European Commission President Ursula von der Leyen said Finland's decision to close its border crossings with Russia over a surge in migrants was a security matter for the whole 27-member bloc to consider.

"We all know how Putin and his allies instrumentalise migrants to test our defences and to try to destabilise us," she said.

"Now Putin is focusing on Finland, and this is no doubt in response to your firm support of Ukraine and your accession to NATO."

Finland shares a 832-mile (1,430km) land border with Russia.

"This is not just about the security of Finland, but it is about the security of the European Union. We are in this together," Ms von der Leyen said after visiting the border in Lappeenranta with Finnish Prime Minister Petteri Orpo.

"We should be more Finnish when it comes to security."

Finland joined NATO in April 2023 in response to Russia's invasion of Ukraine in a major blow to President Putin.

Slovaks angered at their government's refusal to give military aid to Ukraine have raised nearly €2m (£1.7m) in a drive to help supply ammunition to Kyiv.

Although ammo supplies are a pressing need for Ukraine after two years of war, Slovakia has refused to join a plan led by the Czech government to buy hundreds of thousands of artillery rounds for Ukraine.

"When I heard about the Czech government's initiative, I was very pleased to hear that all ways are being sought to help Ukraine defend itself against the [Russian] aggressor, because there is no other way," said Otto Simko, a 99-year-old Holocaust survivor and journalist who helped spearhead the Slovak crowdfunding campaign.

In a video posted on the crowdfunders' YouTube page he said Russia needed to be expelled from Ukraine so "peace can be spoken of on terms that suit Ukrainian independence".

Slovakia halted state military aid to Kyiv last year, arguing the conflict did not have a military solution.

It is "imperative" Ukraine gets more resources immediately to help it fight off Russia's invasion, the US secretary of state has said after a meeting of the Group of Seven (G7) major democracies.

"It needs more air defences, it needs more munitions, it needs more artillery - allies and partners including the G7 countries are committed to delivering on that," Antony Blinken added.

Mr Blinken went on to say that if China wants better relations with Europe it cannot continue helping Russia while it attacks Ukraine, adding that Beijing was the primary contributor to Russia's defence industrial base.

He made the remarks after Kyiv's foreign minister said the G7 had identified "specific steps" needed to help Ukraine in its fight against Russia, Kyiv's foreign minister has said.

Dmytro Kuleba also warned Europe would be engulfed by war if Russia triumphed in its invasion.

"We identified specific steps which Western partners will make to help Ukraine," Mr Kuleba told reporters on the Italian island of Capri, where G7 foreign ministers are meeting.

He said the West had the capacity "to provide Ukraine with all necessary resources as soon as possible to save Europe from a larger war."

Meanwhile, Italian foreign minister Antonio Tajani said the G7 was looking to see if it could use frozen Russian assets held in the West and not just the interest from the funds.

Mr Tajani said there was an established legal basis for using the interest from the frozen funds, but experts were now looking to see if the capital itself could be used to help Ukraine.

The West has frozen some $300bn (£241bn) of sovereign Russian assets, which the UK and US want to be used to pay for the Ukraine war effort - though European Union member states have questioned the legality of such a move.

A Polish man has been arrested over allegations of being ready to help Russia's military intelligence in an alleged plot to assassinate Volodymyr Zelenskyy, Polish prosecutors said.

The man, identified only as Pawel K, was accused of being prepared to pass airport security information to Russian agents and was arrested in Poland on Wednesday, the office of Poland's National Prosecutor said in a statement.

The man was seeking contact with Russians directly involved in the war in Ukraine and was expected to pass on detailed information about the Rzeszow-Jasionka airport in south-eastern Poland, near the border with Ukraine, it said.

The airport is under the control of US troops and serves as a gateway for international military and humanitarian supplies for Ukraine.

If convicted, the man could face up to eight years in prison, the statement said.

It said the arrest was the result of close cooperation with the prosecutors and security services of Ukraine, who tipped them off and provided crucial evidence.

The Kremlin has declined to comment.

It comes after German prosecutors said two German-Russian men had been arrested on suspicion of espionage (see 8.44 post yesterday).

One of them is accused of agreeing to carry out attacks on potential targets including US military facilities in the hopes of sabotaging aid for Ukraine.

Volodymyr Zelenskyy has visited troops near the frontline in the east of Ukraine.

The Ukrainian president joined a paratroopers' medical platoon in the Donetsk region.

"Today - Donetsk region. Visited our defenders who are under treatment," he said on Telegram.

Mr Zelenskyy said he talked with the soldiers and gave them awards, adding: "Thank you for your service and protection of Ukraine! Our country is proud to have such soldiers."

What to expect in Homeland Security Secretary Alejandro Mayorkas impeachment proceedings

easy trip planners news today

WASHINGTON – The House will delay sending articles of impeachment against Homeland Security Secretary Alejandro Mayorkas to the Senate until next week, a move intended to give Republicans more leverage to push for a full impeachment trial as they continue to blast the secretary's handling of the U.S. southern border.

The lower chamber had planned to send the articles on Wednesday, setting up the Senate to start their own process on Thursday.

But Democratic leaders are not expected to allow a full trial against Mayorkas on the Senate floor, so the mid-week transfer would likely have put additional pressure on the chamber to dismiss the impeachment articles quickly so senators could leave town for the weekend.

"To ensure the Senate has adequate time to perform its constitutional duty, the House will transmit the article of impeachment to the Senate next week," Taylor Haulsee, spokesperson for House Speaker Mike Johnson, said in a statement. "There is no reason whatsoever for the Senate to abdicate its responsibility to hold an impeachment trial."

Senate Majority Leader Chuck Schumer, D-N.Y., told reporters shortly afterward that Senate Democrats are still "ready to go" whenever the House initiates the proceedings.

Prep for the polls: See who is running for president and compare where they stand on key issues in our Voter Guide

"We're sticking with our plan," he said. "We're going to move this as expeditiously as possible."

Senate Republicans plan to raise multiple "points of order" on the floor when the House does send over the articles of impeachment they passed earlier this year. A point of order is a procedural move that would drag out the vote and may force Democrats to take politically uncomfortable votes in a high-stakes election year.

Republicans' strategy is unlikely to change the final result, as Democrats are expected to dismiss or essentially stall the impeachment proceedings. However, key centrist senators that typically vote with Democrats could break rank, which would change the equation if all Republicans stick together.

Senate GOP Whip Sen. John Thune of South Dakota said Tuesday that the "overwhelming majority" of Republicans will vote to hold a trial, indicating that there are likely to be at least a few defectors.

The conservative case for holding a trial

A group of seven ultraconservative senators led by Sen. Mike Lee, R-Utah, railed against Democrats' plan to avoid an impeachment trial for nearly an hour on the Senate floor Monday evening, urging their colleagues to vote to hold a full impeachment trial – or face voters' wrath at the ballot box this fall.

"If you're so confident that the charges against Secretary Mayorkas are baseless, then why not hold a trial?" Lee said on the floor Sunday. "This is exactly what it looks like when someone is aware that there is a problem and wants to sweep the problem under the rug... You can't hide this."

The Senate has held a trial for every impeached official unless they died or left office before a trial could be held, Lee told USA TODAY ahead of the speeches. "Tabling it is not just a terrible idea, but it's counter-constitutional."

Sen. Ted Cruz, R-Texas, argued during a press conference Tuesday that Democrats only plan to dismiss the trial to shield vulnerable Democratic senators that are up for reelection this year: "That's who Chuck Schumer is trying to protect from having to hear the evidence and fulfill a constitutional responsibility."

The conservative senators said they would explore the procedural options to make dismissing the trial as politically painful as possible.

"This is the United States Constitution and 240-plus years of precedent. My personal view is that the consequences ought to be commiserate with the action," said Sen. Eric Schmitt, R-Mo.

Senate Majority Leader Chuck Schumer has repeatedly called it a "sham impeachment" that's "a new low for House Republicans."

"House Republicans failed to produce any evidence that Secretary Mayorkas has committed any crime," Schumer said earlier this year, and "failed to present any evidence of anything resembling an impeachable offense."

Why did the Republican-led House of Representatives impeach Mayorkas?

The Republican-led House voted to impeach Mayorkas in February by a count of 214-213, making him the second cabinet secretary in American history to be impeached (the first was nearly 150 years ago .) No Democrats supported the effort, and a few Republicans also voted against it.

House Republicans argued that Mayorkas violated the Constitution by deliberately refusing to enforce border security laws.

The impeachment inquiry in the House "demonstrated beyond any doubt that Secretary Mayorkas has willfully and systemically refused to comply with the laws of the United States, and breached the public trust," said House Homeland Security Committee Chair Rep. Mark Green, R-Tenn., earlier this year.

But Democrats, some Republicans, constitutional law experts and former Homeland Security secretaries have contended that the GOP-led effort uses the impeachment process – typically reserved for conduct considered high crimes and misdemeanors – to settle a policy disagreement about how to address the nation's immigration system, effectively weakening a powerful congressional tool.

"We've taken impeachment and we've made it a social media issue as opposed to a Constitutional one," Rep. Ken Buck, R-Colo., told reporters shortly after announcing he would leave Congress early . "This place just keeps going down, and I don't need to spend my time here."

The debate has come amid surging migration to the southern border, with many people fleeing dangerous conditions and economic uncertainty in central and South America and seeking refuge in the United States.

Immigration has become a major issue in the presidential election this fall. In January, immigration was the top problem cited by American voters in a Gallup poll . A February survey from the Pew Research Center found 80% of Americans feel the government is doing a bad job handling the number of migrants at the border.

Here's what to expect in the Senate

When the House sends the articles of impeachment over to the Senate, Johnson will hold a ceremony to sign them.

The impeachment articles will then be walked across the Capitol building to the Senate by the House impeachment managers, which include Green, Foreign Affairs Committee Chair Rep. Michael McCaul, R-Texas, and Reps. Andy Biggs, R-Ariz., Marjorie Taylor Greene, R-Ga., Ben Cline, R-Va., Andrew Garbarino, R-N.Y., Michael Guest, R-Miss., Clay Higgins, R-La. Laurel Lee, R-Fla., August Pfluger, R-Texas, and Harriet Hageman, R-Wyo.

The articles will be read out loud and the day after, each senator will be sworn in as a juror in the trial.

Then Democratic leaders are likely to move to dismiss the trial, which would only require a majority vote to accomplish.

However, it's not clear exactly whether moderate members of each party might defect from their peers. Asked where they stand on dismissing the trial, multiple senators including Manchin, Sen. Lisa Murkowski, R-Alaska, and Sen. Susan Collins, R-Maine, declined to say on Monday how they'd vote (though Manchin called the impeachment effort " pure crap " in February.)

The Senate could also choose to send the impeachment trial to a committee to prevent it from taking up valuable floor time, which would eventually go back to the full Senate to vote whether to convict.

U.S. issues travel warning for Israel with Iran attack believed to be imminent and fear Gaza war could spread

By Debora Patta , Tucker Reals

Updated on: April 13, 2024 / 5:25 PM EDT / CBS News

Update: Iran launched drone attacks against Israel on Saturday. Read CBS News'  latest coverage here .

Tel Aviv  — Israel is bracing for a worst-case scenario that U.S. officials believe could materialize within just hours — the possibility of a direct attack on Israeli soil by Iran in retaliation for a strike almost two weeks ago that killed seven Iranian military officers. Iran has vowed to take revenge for Israel killing its commanders, who were hit by an April 1 strike on the Iranian embassy in Syria's capital.

Two U.S. officials told CBS News that a major Iranian attack against Israel was expected as soon as Friday, possibly to include more than 100 drones and dozens of missiles aimed at military targets inside the country. Sources have told CBS News the retaliation could include attacks carried out both by Iranian forces, and proxy groups around the region that it has been funneling additional arms to for weeks.

The officials said it would be challenging for the Israelis to defend against an attack of that magnitude, and while they held out the possibility that the Iranians could opt for a smaller-scale attack to avoid a dramatic escalation, their retaliation was believed to be imminent.

Asked Friday how imminent he believes an attack is, President Biden responded, "I don't want to get into secure information, but my expectation is sooner than later." The president urged Iran not to move forward, saying his message to Tehran was: "Don't."

Tehran has not indicated publicly how or when it will return fire, so it's unclear how far Iran's leaders will go. If they decide to carry out a direct attack on Israel, there's fear it could blow Israel's ongoing war against Iranian ally Hamas up into a much wider regional conflict.

With the Iranian retaliation expected at any time, the U.S. State Department on Thursday warned Americans in Israel not to travel outside major cities, which are better protected from incoming rocket fire by the country's Iron Dome missile defense system. The latest guidance noted that travel by U.S. government employees in Israel could be further restricted with little notice as things develop in the tinderbox region.

"Whoever harms us, we will harm them," Prime Minister Benjamin Netanyahu vowed Thursday as he visited troops at an Israel Defense Forces airbase. "We are prepared … both defensively and offensively."

Iran-Burning Flags Of The U.S. And Israel

On Saturday, all U.S. embassies in the Middle East were put on high alert and required to hold emergency action committee meetings. Diplomats in Lebanon and Israel were specifically told not to travel to certain areas within those countries.

Sima Shine, a security expert and former official with Israel's national intelligence agency Mossad, told CBS News it was a dangerous moment for the region, and the "most worried" she has been. She said anxiety over an all-out war was likely just as high "on both sides, in Israel and in Iran."

If Iran does choose to strike Israel directly, it could involve a complex missile and drone attack similar to the one Iranian forces launched against a Saudi oil facility in 2019 .

"They will try to do it on the military or some military asset," Shine predicted. "But the question will be the damage. If there would be many injured people, killed or injured … I think it has the potential for a huge escalation."

Shine stressed, however, that she still believes neither side actually wants a regional conflict.

U.S. "really trying to avoid war"

The U.S. sent a senior general to Israel this week to coordinate with the close American ally on any response it might make to an Iranian attack. Speaking Friday on "CBS Mornings," America's top military officer said, "we're really trying to avoid war."

"This is part of the dialogue that I have with my counterparts within the region, to include the Israeli chief of defense, who I talked to yesterday," said Joint Chiefs chairman Gen. Charles Q. Brown, Jr., adding that the U.S. military was "doing things not only to prevent a war, but at the same time, one of my primary things is to make sure all the forces in the region are protected."

"My role, as the chairman of the Joint Chiefs, is to plan and prepare," Brown said. "That's one thing we do very well."

Brown's Israeli counterpart, Chief of the General Staff Lt. Gen. Herzi Halevi, "completed a comprehensive situational assessment on the readiness of the IDF for all scenarios," Israel's military said Friday.

"The IDF is very strongly prepared, both offensively and defensively, against any threat," Halevi was quoted as saying in the statement. "The IDF continues to monitor closely what is happening in Iran and different arenas, constantly preparing to deal with existing and potential threats in coordination with the United States Armed Forces."

The IDF said the visiting U.S. general, Central Command chief Gen. Michael Erik Kurilla, was taking part in the IDF's situational assessment.

The dilemma for Iran, said Israeli expert Shine, is to figure out how to deliver its promised response to Israel's attack in Syria, but in a way that does not lead to further escalation. Likewise, Shine said Israel could choose to show restraint when it responds to whatever Iran eventually does.

If either side gets the balance wrong, the consequences for the region, and even the world, could be dire.
