Medical Tourism: Contemporary and Future Issues - Troublesome or not?
Depending on who you believe medical tourism is either the new wave of cheap international health care or a gross exaggeration of the extent of international travel flows. Potentially both are true! The presentation will focus on what we do and do not know about the extent of international medical tourism and its potential implications. A key issue will be that in many jurisdictions the actual extent of medical tourism is virtually unknown - a significant with obvious implications for managing any issues that may arise as well as good policy making. The presentation will move through this relative research vaccuum in an attempt to highlight the potential risks to public and personal health posed by medical tourism and some of the ethical issues involved as well as try to highlight some potential future directions for medical tourism, including the potential future "perfect storm" of increased tourism and international medical mobility, environmental change and increased antibiotic resistance. The presentation largely builds on a forthcoming chapter in the The Elgar Handbook on Medical Tourism and Patient Mobility. Bio: C. Health and Medical Tourism: Kill or Cure for Global Public Health? Abstract Purpose – The major purpose of this introduction to the special issue of Tourism Review on health and medical tourism is to outline some of main issues that exist in the academic literature in this rapidly developing field. Design/methodology/approach – The paper reviews relevant health and medical tourism and cognate literature. Findings – The paper identifies some of the interrelationships between different areas of health and medical tourism, including wellness and wellbeing tourism, dental tourism, stem-cell tourism, transplant tourism, abortion tourism and xeno-tourism. Key to defining these areas are the relationships to concepts of wellness and illness and the extent to which regulation encourages individuals to engage in cross-border purchase of health services and products. Key themes that emerge in the literature include regulation, ethics, the potential individual and public health risks associated with medical tourism, and the relative lack of information on the extent of medical tourism. Social implications – The development of international medical tourism is demonstrated to have potentially significant implications for global public health. Originality/value – The paper covers an extensive range of academic literature on international medical tourism which indicates the different approaches and emphases of research in different disciplines as well as the ideological and philosophical differences that exist with respect to health medical tourism. The paper also notes that some of the individual and public health risks of medical tourism are not usually incorporated into assessments of its potential economic benefits. Key words mobility, spa tourism, wellness tourism, health tourism, risks Paper type General review This paper serves as the introduction to the special issue of Tourism Review on health and medical tourism. Contemporary medical tourism builds upon long-standing links between travel and the pursuit of physical, mental, and spiritual well-being. It is produced today through the combination of, on the one hand, shifts in concentrations of accessible medical expertise and technologies around the world and, on the other, increased medicalization, care commodification, and perception of bodies as improve-able, malleable property. This chapter considers the impacts of this transnational phenomenon on contexts both generating and receiving medical tourists as well as the lesser-explored virtual and transit spaces between them and the methodological challenges that researchers face in studying medical tourism.
Daryna Kostiuchyk
'Medical tourism' is a term used to refer to a travel activity that involves a medical procedure or to activities that promote the wellbeing of the tourist. This paper utilises the factors in Porter's diamond to evaluate, analyse and discuss the growth and development of medical tourism. Unacceptably long hospital queues and high costs of medical procedures in western society have created a demand for medical tourism. Hence, as this new global product gains popularity, increased monitoring and greater accreditation of this service is required to keep pace with an increase in its international demand.
SUMMARY 1. The global growth in the flow of patients and health professionals as well as medical technology, capital funding and regulatory regimes across national borders has given rise to new patterns of consumption and production of healthcare services over recent decades. A significant new element of a growing trade in healthcare has involved the movement of patients across borders in the pursuit of medical treatment and health; a phenomenon commonly termed‗ medical tourism '.

Medical, health and wellness tourism and travel represent a dynamic and rapidly growing multi-disciplinary economic activity and field of knowledge. This research responds to earlier calls to integrate research on travel medicine and tourism. It critically reviews the literature published on these topics over a 50-year period (1970 to 2020) using CiteSpace software. Some 802 articles were gathered and analyzed from major databases including the Web of Science and Scopus. Markets (demand and behavior), destinations (development and promotion), and development environments (policies and impacts) emerged as the main three research themes in medical-health-wellness tourism. Medical-health-wellness tourism will integrate with other care sectors and become more embedded in policy-making related to sustainable development, especially with regards to quality of life initiatives. A future research agenda for medical-health-tourism is discussed.

The opening years of the 21st century have been characterized by a variety of technological, social, political and economic factors that have seen the disintegration of the meaning and importance of international borders. From the power of the Internet to link disparate populations, to the consolidation of blocs of politically intertwined nations such as the European Union (EU), and the rise of affordable international travel, forces have been combining to bring about a globalized world economy that was only theorized in decades past. An obvious facet of this new globalization is the permeability of borders to the movement of consumers seeking a variety of medical services, and providers willing to accommodate, if not also profit, from this demand.

MEDICAL TOURISM
Transcript: MEDICAL TOURISM Medical tourism is a trend that has emerged within the last half century. More than 500,000 residents of the United States travel overseas annually to get a medical procedure done. The concept of travelling from ones motherland to a foreign nation is the steadily increasing in popularity concept of MEDICAL TOURISM. POSITIVE ASPECTS Cost of medical procedures in the United States are increasing dramatically. At the same time the quality of healthcare in foreign nations is on a steady improvement. The uninsured American residents can get good quality medical services at lower prices in over-sea countries. Many procedures are not covered by insurance companies and or Medicare/Medicaid in the United States. Medical tourism allows patients a vacation in new countries, while they are getting treated. The Joint Commission International sets standards for foreign hospitals and clinics to meet, before they can be accreditted and be recommended to patients thinking of travelling abroad. Many procedures such as organ transplants are available in International Countries. There is a long organ transplant waitlist in the United States. Reproductive technology was considered illegal/unethical in the United States, but was practiced overseas. if the rate of medical procedures conducted in foreign nations continues to rise – there will be a gradual decline in the employment rate for medical professionals within the United States. medical malpractice suits: regulations are not followed as closely within foreign countries. If any unforeseen events were to occur – it might not be compensated for. A patient's insurance may or may not provide complete coverage, especially if traveling abroad because of the numerous risks involved Postoperative treatments are critical – with medical tourism it is difficult to make arrangements after the operation Traveling for two overseas can accumulate to be very costly and overwhelming for a patient and their family First hand viewing of the hospital usually cannot occur ahead of time, because of massive costs associated with traveling It is a major risk of faith that the foreign hospital or clinic will meet up to par with the patient’s expectations from the webpage As a team we believe that Medical tourism is a beneficial decision. One can search through the Joint Commission International database, to see where they can receive their medical treatments as early as possible It puts the least amount of burden on a patient’s bank account. Many innovative and futuristic ideas can be tested within these countries, i.e.: stem cell research Patients favor the idea that they can also vacation afterwards in a different country. Residents of the US are spending less money for the same goods and services, because they have a higher purchasing power in several countries We believe that human life is worth a risk so why should one settle for nothing, when there is an abundance of options all over the world? Task What is Medical Tourism? OUR OPINION NEGATIVE ASPECTS Deepa Jois & Malvika Saharya Our opinion in continuation Downfalls Continued

Healthcare & Medical Tourism Components
Enhancement • ‘Enhancement’ procedures are carried out mainly for aesthetic purposes(much of this work is non-disease related unless disfigurement is caused by disease). These are: • All cosmetic surgeries • Breast surgeries • Facelifts, • liposuction and cosmetic dental work.
Wellness(Spa/Alternate therapies) • The ‘wellness’ segment of medical and healthcare tourism promotes healthier lifestyles. These products can include: • Treatment in spas • Thermal and water treatment • Acupuncture and Aromatherapy • Beauty care, facials, exercise and diet • Herbal healing, homeopathy, massage • Yoga and naturopathy.
Reproduction(Fertility) • Under this component, there are patients who seek fertility-related treatments such as in vitro and in vivo fertilization and other similar procedures. • At times, potential parents travel for the purposes of adopting children because the legislation and supply of babies for adoption is easier in host countries.
Hospital Accreditation • Accreditation defined as "A self-assessment and external peer assessment process used by health care organisations to accurately assess their level of performance in relation to established standards and to implement ways to continuously improve“. • Fundamentally healthcare and hospital accreditation is about improving how care is delivered to patients and the quality of the care they receive.
Accreditation Schemes • Trent Accreditation Scheme (based in UK-Europe) • QHA Trent Accreditation, based in the UK • Joint Commission International, or JCI (based in USA) • Australian Council for Healthcare Standards International, or ACHSI (based in Australia) • Accreditation Canada (formerly the Canadian Council on Health Services Accreditation or CCHSA -based in Canada) • Accreditation of France based in Paris, France. • NABH(National Accreditation Board for Hospitals and Health Centers- based in India)

Medical Tourism Provider • A Medical Tourism Provider is an organization or a company which seeks to bring together a prospective patient with a service provider, usually a hospital or a clinic. • Some such organisations and companies specialize in certain areas of healthcare, such as cosmetic surgery, dentistry or transplant surgery, while others are more generalized in their approach. • Medical quality standards vary around the world, and international accreditation is relatively new. • For these reasons, Medical tourists look to health tourism providers to provide information about • Quality, • Safety and • Legal issues.
Various issues for Medical Tourists • Non Medical issues • Prices and how to pay • Hotels(checking in) • Language issues • Ethics • Medical issues • Availability of techniques • Pre-travel health issues, such as antimalarial therapy and relevant immunizations • Medico-legal issues
Major Players in India • Following are major players in the Medical Tourism industry. • Escorts • Apollo Hospital • Max Healthcare • Indraprastha Apollo Hospital • Hinduja Hospital • Jaslok Hospital • Fortis • Wockhardt
Medical Tourism : SWOT approach • Through such an analysis, the Strengths and Weaknesses existing within the medical tourism can be matched with the Opportunities and Threats operating in the Indian environment so that an effective strategy can be formulated.
Strengths • A strength is an inherent capacity which an organization can use to gain strategic advantage. • Quality and Range of Services • Affordable costs • Vast supply of qualified doctors: • Strong presence in advanced health care • International Reputation of hospitals and Doctors. • Diversity of tourism destinations and experiences
Weakness • A weakness is an inherent limitation or constraint which creates strategic disadvantages. • No strong government support/initiative to promote medical tourism. • Low coordination between the various players in the industry- airline operators, hotels, and hospitals. • Lack of uniform pricing policies across hospitals. • Customer Perception as an unhygienic country • Political problems etc
Opportunities • An opportunity is a favorable condition in the service industry which enables it to consolidate and strengthen its position • Increased demand for healthcare services from countries with aging population (US, UK) • Fast-paced life style increases demand for wellness tourism and alternative cures. • Shortage of supply in National Health Systems in countries like UK, Canada. • Demand from countries with underdeveloped healthcare facilities. • Demand for retirement homes for elderly people especially Japanese. • Personal touch by the doctors in India. • Traffic system is well developed and easy to go from one country to another country. • Medicines and lab-our cost is low as compared to developed countries.
Threats • A threat is an unfavorable condition in the industry’s environment which creates a risk for, or causes damage to, the industry. • Strong competition from countries like Thailand, Malaysia and Singapore. • Lack of international accreditation – a major inhibitor. • Overseas medical care not covered by insurance providers. • Under – investment in health infrastructure. • Exploitation of tourists by illegal money changers. • Terrorism • Rise of Resistant strains (NDM-1)
Competition • Thailand, Singapore, Malaysia, India and Philippines are the major destinations in the Asian medical tourism market. • Thailand is more popular among Western European medical tourists for cosmetic surgery. • Singapore and India specialize in complex procedures with India having a cost advantage and Singapore a technology advantage.
Advantage of Medical Tourism In India • The biggest advantage of medical travel in India is the low cost health care service • Technological advancements and highly educated doctors • Availability of latest medical instruments • High health care standards offered • No language barrier as English is widely spoken across India. Moreover many hospitals in India hire native translators for patients from African countries so that they can communicate effectively. • Relaxed visa rules • Visa on India arrival for patients from specific countries • Availability of low cost generic medicines
Disadvantage • Medical Malpractice • Insurance Coverage • Post Operation Recovery • Travel Cost
Role of Government • The role of Indian Government for success in medical tourism is two-fold • Acting as a Regulator to institute a uniform grading and accreditation system for hospitals to build consumers’ trust. • Acting as a Facilitator for encouraging private investment in medical infrastructure and policymaking for improving medical tourism. Investment facilitator: • Recognize healthcare as an infrastructure sector, and extend the benefits under sec 80-IA of the IT Act. • Should actively promote FDI in healthcare sector • Providing low interest rate loans, reducing import/excise duty for medical equipment • Facilitating clearances and certification
Government’s Role • Tourism facilitator: • Reduce hassles in visa process(M-Visa) • Follow an Open-Sky policy to increase inflow of flights into India • Create Medical Attachés to Indian embassies that promote health services to prospective Indian visitor • Private Sector Role Increased participation in building infrastructure Integrate Horizontally Joint Ventures / Alliances
Conclusion • The concept of medical tourism is appealing to anyone who is interested in high quality and affordable healthcare. • The medical tourism phenomenon is gaining popularity and the number of people going abroad for treatment increases rapidly every year. • With many medical tourism benefits, advancements in technology and improvements in healthcare standards within developing countries, • it is likely that the advantages of medical tourism will provide a striking economical solution to many healthcare problems
