Omicron and Holiday Travel: How to Strategize

Even as the variant surges, the seasonal travel rush seems unstoppable, but there are steps you can take to travel more responsibly and mitigate the health risks for yourself — and others.

Taylor Allen wanted to be a responsible traveler, but she was finding it difficult.

Late last week at least seven people Ms. Allen knew in Brooklyn posted on Instagram that they’d tested positive for the coronavirus. She had not seen any of them in person. But after developing an intense headache and runny nose on Friday, she canceled her Saturday morning flight to Jacksonville, Fla., where she was planning to see her parents and grandparents.

Two at-home tests — one Friday and one Saturday — came back negative. But Ms. Allen, 22, who is fully vaccinated but not yet boosted, wanted more official assurance before she rebooked her trip. On Sunday evening, long after her scheduled appointment at an urgent care clinic in Crown Heights, an employee told her and the 30 or so other people waiting for tests in the bitter cold that they’d have to come back at 8 a.m.

“I really don’t want to put anyone in danger,” said Ms. Allen, who left the clinic with plans to return again the next day.

Even as the number of coronavirus cases is skyrocketing in some parts of the country , largely driven by the Omicron variant, the holiday travel rush appears unstoppable. On Friday, Los Angeles International Airport reported its busiest day since early 2020, and on Sunday, 2.1 million people passed through airports in the United States, nearly twice as many as at this time last year.

For those who are determined to keep their travel plans, figuring out how to do so responsibly has never been more confusing. Part of the problem is that testing has been hard to obtain in a timely way, particularly in hard-hit cities like New York. Another key challenge is that many people plan to stay in a house with fully vaccinated friends and family. Now, they are learning that vaccination is far from a guarantee that they won’t infect one another. So what can travelers do?

1. Get a booster

Only one in six Americans have received a booster, according to the Centers for Disease Control and Prevention. Fully vaccinated individuals without a booster are at least twice as likely to test positive as those who received a booster.

If you plan to travel over the coming weeks and months, and you’re already fully vaccinated, one of the best ways to be a responsible traveler is to get a booster, said Jeffrey Kahn, the director of the Johns Hopkins Berman Institute of Bioethics.

In terms of timing, the data show that the optimal immune response comes about two weeks after the booster, according to Dr. Wafaa El-Sadr, a professor of epidemiology and medicine at the Columbia University Mailman School of Public Health in New York. But many will see some protection within a few days, other experts noted, so getting a third shot today could still benefit those traveling over the holidays.

2. Consider the worst-case scenario

When deciding what’s responsible in terms of holiday travel, Kelly Hills, a co-founder of Rogue Bioethics, a consulting firm in Boston, advises thinking about “moral injury” and asking whether you are mentally prepared for the consequences if you infect a vulnerable person.

That doesn’t have to translate into canceling plans, but it may encourage you to wear an N95 instead of a homemade mask on a plane or to take a test even though it’s a hassle. If you are indoors, unmasked around many people in the days leading up to the trip, you may also want to pay extra to book a separate house or motel room, rather than staying with family or friends.

“‘I don’t want to be a spreader’— that should be the motto today,” said Leonard J. Marcus, the co-director of the National Preparedness Leadership Initiative at Harvard University and the director of an initiative focused on public health on flights.

Dr. Marcus said that though he’s not aware of any data suggesting children are likely to become infected on planes, he advises parents not to fly with unvaccinated children — if possible — until more is known about Omicron.

“If it were my grandchildren, I would postpone,” he said. In general, if someone is wearing a proper mask on a plane, the risk of being infected should be low because the ventilation system is so good, he said.

Dr. Anthony Fauci, the nation’s top infectious disease official, told CNBC that he feels safe having his adult children fly to see him over the holidays. He also noted that they are vaccinated.

3. Test as close to the gathering as possible

Testing in many parts of the country is challenging right now.

“On a one to 10 scale of hard, it’s a 10,” said Mary Mathurin, 51, outside a testing site in Brooklyn on Sunday evening. As she waited for her name to be called, her cellphone emitted hold music from a call with another facility that had yet to send her P.C.R. results from several days earlier. After around 70 minutes, the call dropped. A few minutes later, a patient care assistant at the Brooklyn site told her the site could not accommodate her. She was supposed to fly to St. Lucia the next morning and was unsure what she was going to do.

Many pharmacies and online retailers have sold out of at-home tests. The White House is planning to make 500 million free at-home tests available, but that won’t happen until January. For those who do manage to get a kit, use it as close to your departure date as possible, several experts said.

“The closer you are to the event, the better and more accurate it will be,” said Dr. Lin H. Chen, a professor at Harvard Medical School and the director of the Mount Auburn Hospital Travel Medicine Center in Cambridge, Mass.

Dr. Chen suggested taking an at-home antigen test the day of the gathering. (If a person tests positive at any point, she advised not going to the event and getting a P.C.R. test for confirmation.) If people are staying in a house together for an extended period, testing periodically throughout their stay is advisable, Dr. Chen said. This is particularly important if someone is not vaccinated or boosted, or has been exposed to someone who has tested positive, other experts noted.

Yes, everyone is confused

Ms. Hills, the bioethicist , said that it’s understandable that many people are confused by having to make what should be public health decisions.

“We should be getting more guidance,” she said, noting that many state and federal agencies offer different advice.

At the testing site in Brooklyn, several travelers echoed this point and lamented that public health authorities were not making what they considered responsible travel — getting tested before visiting family — easier.

Compounding these frustrations, some travelers said, is the sense that the burden is on them to figure out what is socially responsible and epidemiologically safe and then to convince their family and friends of the policies they have come up with. One woman, who declined to use her name because she didn’t want her family to identify her, said she no longer feels comfortable flying with her 2- and 3-year-old children after learning over Thanksgiving that her own family members would fly even if they tested positive.

Rather than fight with them about what’s appropriate or worry that the people sitting next to her share her family’s approach — and could infect her or her children, leading them to infect her father — she is going to stay home this Christmas.

It can help you identify areas you can potentially save money on trip expenses. Whether it’s choosing more budget-friendly hotels, walking or taking the bus instead of taxis, or prioritizing your top not-to-miss activities.

You can also start with your budget.

Then play around with the length of your trip, how much hotels, flights etc will cost to help you determine your price range for each travel expense as well.

Enter all your estimated expenses you’ve gathered during your vacation research.

Make sure it’s actually a realistic travel budget. For a  family trip with young kids, you’ll also need to factor other things like diapers, baby wipes etc.

• Travel Budget : Enter your overall vacation budget you have for this trip.
• Number of Travellers: Enter the number of people you are paying for on this trip
• Number of Nights: Enter the number of nights you will need accommodations for. This will be used to calculate hotel costs.
• Number of Vacation Days: Enter the number of days you will be on vacation. Include travel days to be safe . This will be used to calculate your overall food & drink, daily transportation and activity costs.
• Total Flight Costs: Enter the estimated total of flight costs for everyone travelling. Be sure to include any baggage fees.
• Transportation Costs: Enter the total transportation costs for the trip. This is for any major transport costs that aren’t flights like trains, city transfers, car or RV rentals.
• Daily Transportation Costs: Enter your anticipated daily transport costs. This is for things like taxis, buses, Ubers, parking costs etc, you might need to get around every day.
• Hotel Costs (Per Night): Enter the amount your accommodations will cost each night. Don’t forget to include any tips for housekeeping staff etc. you may need to pay.
• Total Tour Costs: Enter your overall total budget or cost for tours, including guide tips.
• Activity Costs (Per Day) : Enter the amount you expect to spend on activities daily (do not include tours). These are for things like seeing movies, souvenirs, shopping, trips to a rec centre / pool, bowling, seeing a hockey game or a concert etc.
• Food & Drink Costs (per Day): Enter your overall daily food & drink budget or expected costs for everyone. Don’t forget to include tips.
• Miscellaneous Expenses: Enter an amount of money you will have overall for unexpected expenses or miscellaneous costs that pop up. Be sure to also include extra travel expenses not covered elsewhere.
• Travel Insurance: Enter the overall amount of money you will pay for travel insurance for everyone. This is for things like travel health insurance, trip cancellation insurance etc.

Currency Note: While the output is in dollars (uses the $ symbol), you can really use any numerical currency you’d like. Just ignore the $ symbol.

Once you hit the calculate button, you will get 4 pieces of info based on the numbers you provided:

• Total Vacation Costs : This is the estimated cost for your vacation based on the info you provided.
• Over Budget / Under Budget: This is the amount you are either over or under budget. If your expenses are over your vacation budget, it will say Over Budget and the number will be negative. If it says Under Budget, that is the amount you are under your estimated budget.
• Travel Costs Per Day: This is the amount of money your trip will cost you each day.
• Cost Per Person Per Day: This is the amount of money your trip will cost you per person, each day.

Why You Need A Travel Budget

Listen, I am all for spending a little extra to do things our family doesn’t get to do at home. 

But there is nothing more anxiety-inducing than running out of money on a trip, or having to pay off huge credit card bills at 28%  interest because you didn’t plan ahead.

This is where a vacation budget can really help. 

It doesn’t mean you can’t have fun or spend money.

Creating a budget will actually help you be able to afford your dreams. Plus be able to relax & have fun on your vacation knowing you won’t be in debt when you get back home.

How To Set Your Travel Budget & Determine Expenses

1. determine your overall travel budget.

First things first. Decide on your overall vacation budget or the money you are willing to spend on this trip.

When determining your overall vacation budget, you’ll want to consider a few things:

• How much money are you willing to spend on this trip?
• How much have you already saved?
• How long do you plan to go for? (more or less time can impact your budget)
• How many people will be travelling? (some places like Europe limit 4 people to a hotel room, this can significantly impact cost)
• When do you want to travel and how much time do you have to add to your vacation fund?
• How much extra can you afford to put away every week / month on top of your regular bills & household expenses?
• Is there any way you can decrease household expenses, or increase your income to add to your vacation fund faster?

Just make sure it’s a realistic budget. It doesn’t do anybody any good to use too low or too high numbers as a starting point. 

2. Do Your Research

The type of trip you want to take, the length and the type of activities you want to do, will really impact your budget.

For example a week long trip to Thailand will be much different than a week in Japan, or a trip to Disneyworld.

Do some intial research to give you an idea of what your basic travel expenses will costs, so you can have ballpark figures to enter in the travel cost calculator.

You can always google average costs of living / travelling at your destination.

Careful planning is an important part of any travel budget. 

3. Find Ways to Cut Costs

Unless you have a rich benefactor, or suddenly won the lottery, it’s a good idea to consider ways you can cut costs, save money and use your travel budget wisely.

General Money Saving Tips

• Travel off season or peak season: Not only is it usually much cheaper, there’s usually fewer tourists.
• Use a credit card with no foreign transaction fees : Those transaction fees can really add up. Also consider if travel credit cards with points are for you.
• Eat where the locals eat : Avoiding the touristy restuarants, and eating where the locals eat will not only save you lots of money, it will give you a more authentic look into their local cuisine and culture.

• Consider using travel agents: Sometimes a travel agent can find you a great deal, with much less work on your part. 
• Adjust your travel style: Luxury resorts may be out of your price range. Road trips, camping trips can be just as memorable for for a family vacation.
• Plan free activities : Pick some free activities to do in between your more expensive tours, theme parks, & day trips to ancient ruins.

Can You Skip Ozempic While on Vacation?

One of the side effects of type 2 diabetes drugs like Ozempic and Mounjaro is increased satiety, or a feeling of fullness after meals—something that can make indulging in vacation treats challenging. It's also part of the reason why these drugs are prescribed off-label for weight loss.

Some social media users say that they’re skipping doses of these drugs while vacationing to enjoy more food, not have to worry about the injections, and drink more alcohol (some people report that Ozempic kills their appetite for booze).

But there are ramifications to missing a dose or two of Ozempic, Mounjaro, or Wegovy.

John Buse, MD, PhD , director of the Diabetes Center at the University of North Carolina Chapel Hill, told Verywell that in general, taking a pause from such medications when you’re on vacation is “a particularly bad idea.”

Buse explains that for people who are prescribed these drugs for medical reasons, like for type 2 diabetes management or weight loss, following the medication guidelines is very important. Not only do you increase your chance of side effects when you restart the medications after a pause, but the drugs should be taken on the correct dosing schedule so they work as effectively as possible. 

“Taking medication on an on-and-off basis—because you think that’s better than taking it as prescribed—is just not a good idea,” he said.

What Happens When You Skip a Dose of Ozempic? 

Novo Nordisk, the drugmaker of Ozempic, says that the medication should be taken once a week, on the same day every week, exactly as prescribed by your healthcare provider. If a dose is missed, the drugmaker says to take the missed shot as soon as possible within five days. Past the five-day mark, patients are advised to take the next dose on the regularly scheduled day.  

Buse explained that when patients start taking injectable GLP-1 agonist drugs like Ozempic and Wegovy, they can experience side effects such as nausea, vomiting, constipation, and diarrhea. These side effects often go away with time, once the patient gets used to the medication. 

But if someone stops for a period of time and then starts up their medication again, they're at risk of those initial side effects coming back, Buse said. This is particularly true for people who had severe side effects when they first started on the medication.

“If a patient of mine asked specifically about going on vacation, and is thinking about stopping the drug while on vacation, I would strongly recommend that they not do that,” Buse said. “And frankly, I would ask the question, ‘Why has this idea occurred to you?’”

For patients who rely on semaglutide (Ozempic and Wegovy) or tirzepatide (Mounjaro) for lowering blood sugar levels and increasing insulin sensitivity, stopping the medication isn’t wise for health purposes—even if the patient wants to get their appetite back. In fact, Buse said not taking the medication on holiday might make patients feel out of control with their eating.

“Blood sugar may not be as well-controlled if you skip doses, and that could be concerning for someone who has diabetes,” Vijaya Surampudi, MD , an assistant professor of medicine in the division of human nutrition at UCLA, told Verywell.

What’s more, taking a break from these medications without consulting a healthcare provider can warrant necessary dosing changes when the patient starts up again. 

It’s important to note that only those who are prescribed these drugs should be taking them. The Food and Drug Administration (FDA) has only approved Ozempic and Mounjaro for type 2 diabetes, whereas Wegovy has been approved for weight loss. Ozempic and Mounjaro are being prescribed off-label for weight loss.

“There’s a lot of people who are taking these drugs almost recreationally,” Buse said. “If you feel like you don’t need the medication [on vacation], because you don’t have the problem, then you probably shouldn’t be on the medication.”

How Long Can You Stay off Ozempic and Maintain Your Weight? 

Buse said the half life of these drugs is about a week. As these medications help control appetite by promoting feelings of satiety, skipping doses will lessen that effect. Seven or eight days after their last dose, patients can expect to feel a bit hungrier than they did two or three days after taking the medication, he said.

Patients can still have a bit of the drug left in their system for about three to four weeks after stopping it, Buse added, but it’s not enough to control appetite. If appetite increases, patients might eat more than they’re used to, which could lead to weight gain.

Surampudi said that patients might start to notice they’ve put on weight shortly after stopping the medication.

“Weight regain can happen pretty quickly with these medications,” she said. “It can be different for everyone, but we’ve seen it as soon as a few weeks; people feel like a couple pounds are creeping back on because their appetite is coming back.”

How to Resume Taking Ozempic After a Break

Depending on how long a patient takes a pause from their medication, they might need to start again on a lower dose, Buse said. This is because drugs like Ozempic or Mounjaro are started at lower doses before a patient is gradually increased to a full dose.

Patients on Wegovy, for example, typically start a dose of 0.25 milligrams once weekly for four weeks before upping the dose in intervals to reach 2.4 mg.

If someone only misses one weekly injection, it might be alright for them to take their next dose as usual, Buse said. But for people who have missed three or more weeks, they’ll likely need to start up again on a lower dose to avoid the side effects and readjust to the medication being in their system.

“Just be prepared that you may have more nausea, vomiting or diarrhea, depending on how bad that [side effect] experience was for you in the beginning [of starting the medication],” he said. “If it was really bad in the beginning, maybe we want to restart with a lower dose—which means a new prescription.”

Drugs like Ozempic or Wegovy are often intended to treat chronic health conditions and are meant to be taken long-term—not started and stopped. If someone needs to manage their weight or diabetes, that need won’t change just because they go on a holiday.

Surampudi compares these medications to other drugs that help patients live longer, healthier lives.

“Once your blood pressure is controlled, it’s not like we stop that medication,” she said. “That’s the way I like to think about this when I explain it to people: Just because your weight is controlled doesn’t mean we stop the medicine.”

What This Means For You

Taking a break from diabetes or weight loss drugs like Ozempic while on vacation isn’t advisable, according to health experts. Instead, patients should continue with their regular dosing schedule in order to prevent any negative health outcomes or side effects.
