Washington, DC Travel Medicine

The Travelers' Clinic at The GW Medical Faculty Associates is dedicated to providing the highest quality and most complete travel medical service in the District of Columbia and the surrounding metropolitan area. We are a Certified Yellow Fever clinic authorized by the Public Health Service to validate certificates of vaccination for immunizations.

Travelers’ Clinic is self-pay. There is a consultation fee plus the cost of any vaccinations you receive at your appointment.

Our medical staff has up-to-date information on vaccination requirements and recommendations for most travel destinations. Immunization requirements can vary depending on the country and may be dictated by government regulation. You should obtain counseling by an MFA medical professional far in advance of your departure date whenever possible. Patients are seen by appointment only and are encouraged to make an appointment at least four weeks prior to traveling.

The Travelers' Clinic clients have access to our infectious disease providers located in the same building. We are conveniently located one block from the Foggy Bottom Metro Station.

Welcome to the Homepage of The Washington Travel Clinic, the private practice of Ziad Akl, MD. Dr. Akl is a physician specializing in Infectious Diseases and Travel Medicine. The Washington Travel Clinic serves the DC metro area and offers all travel vaccines.

Planning a trip outside the United States? Travel clinics are a great resource for getting pre-travel health advice and vaccines for travelers. Travel clinics have doctors and nurses who specialize in travel health and vaccines. They may also stock vaccines that doctor’s offices and pharmacies don’t have.

The Geographic Medicine Center (GMC) is engaged in clinical services, research, and teaching/educational activities in the areas of tropical diseases/emerging infections, vaccine preventable diseases, and travel health.

We believe global travel is a rewarding experience, revealing to us new people, places, and activities. To maintain health when traveling to particular locations, pre-travel interventions may be necessary. Upon return from travel, treatment may be necessary for illness and infections encountered during travel. Our recommendations are based on current Centers for Disease Control and Prevention (CDC) and World Health Organization (WHO) travel notices and guidelines.

Our clinic offers pre and post-travel services for infectious diseases related to travel for adults over the age of 18. Persons younger than 18 should seek care through Pediatric Infectious Diseases .

We take into account your current health status, medications, and planned activities to develop a personalized pre-travel plan for prevention and self-treatment while abroad.  We offer pre-travel recommendations related to:

At Your First Visit

At your initial consultation, we will:

  • Review your health history, including immunizations
  • Review your travel plans
  • Complete a risk assessment
  • Discuss current CDC travel notices
  • Discuss personal prevention behaviors to reduce risk
  • Provide you with prevention recommendations to prevent illness or to self-treat if illness occurs while traveling
  • Recommendations may include immunizations, medications, and behavioral interventions
  • Provide you with written Vaccine Information Sheets. Please read them and ask questions prior to receiving immunizations.
  • Provide you with CDC-WHO International Certification of Vaccination or Prophylaxis

Please note the following:

  • We are a consultative   service.   We do not provide immunizations independently of pre-travel office visits.
  • For routine immunizations in need of updating, you will be referred back to your Primary Care Provider for administration of the vaccine.
  • There is a charge for replacement of lost vaccination certification cards

Insurance and Payment

  • Please check with your insurance prior to consultation. Often, insurances will not cover travel related visits or vaccines.
  • Payment by credit card, debit card, or flexible spending account card is expected at time of service, unless there is an existing contract with your sponsoring organization.
  • If your insurance does cover the consultation, we will submit the visit charges to your insurance company. Should any portion of your visit be covered by your insurance plan, Johns Hopkins will reimburse you.

Our physicians and staff are trained to manage a wide variety of infections.  Patients are seen only by referral from their Primary Care Provider.

Staffed by infectious disease experts with specialized training in travel and tropical disease care, our travel clinic focuses on various diseases most commonly found outside of the United States, such as:

Leading our team are Obinna Nnedu, MD , medical director of Travel Medicine Clinic, and Poppy Markwell, MD . Both are certified in travel and tropical medicine through the American Society of Tropical Medicine & Hygiene, and Dr. Nnedu is also certified in travel medicine through the International Society of Travel Medicine. Together, they diagnose travel-related conditions impacting your health and develop a treatment plan to help you recover.

In most cases, medication is the primary therapy. When necessary, other Ochsner specialists step in to provide surgery and other treatment.

Is a frozen embryo a child? The Alabama court ruling that's rocked America's IVF clinics and reproductive sector

Sperm injection process

When a hospital patient wandered into one of Alabama's fertility clinics in 2020, they caused a chain of events that has left the state's IVF sector, and those using it, reeling. 

The patient, according to court documents, got into the clinic in the city of Mobile through an unsecured doorway before managing to remove several frozen embryos.  

But as the patient picked them up, the subzero temperatures at which they were being stored caused a freezer-burn to their hand. 

The embryos were dropped onto the floor and destroyed.  

Three couples affected by the incident brought lawsuits against the clinic under legislation, first enacted in 1872, that allows parents to recover damages for a child's death. 

At the centre of the case was a key question: Is a frozen embryo considered a child under the state's law?

In a decision that is now reverberating across the country, Alabama's highest court ruled it was.

It's caused several IVF clinics to suspend their services, leaving patients to navigate enormous uncertainty. 

The decision has also prompted broader questions about whether IVF could become America's next battleground over reproductive rights following the end of Roe v Wade . 

Why embryos are now considered 'unborn children' in Alabama

The majority opinion of the Alabama Supreme Court argues "unborn children" have long been considered to be covered by the state's Wrongful Death of a Minor Act. 

Justice Jay Mitchell wrote:

"The central question … is whether the Act contains an unwritten exception to that rule for extrauterine children – that is, unborn children who are located outside of a biological uterus at the time they are killed. "Under existing black-letter law, the answer to that question is no: the Wrongful Death of a Minor Act applied to all unborn children, regardless of their location." 

In his concurring opinion, which included excerpts from the Bible, Chief Justice Tom Parker pointed to Alabama's constitution, which declares a state policy to "recognise and support the sanctity of unborn life":

"Human life cannot be wrongfully destroyed without incurring the wrath of a holy God, who views the destruction of His image as an affront to Himself.  "Even before birth, all human beings bear the image of God, and their lives cannot be destroyed without effacing his glory." 

Alabama-based family lawyer AshLeigh Meyer Dunham, who used IVF to conceive her daughter, says she and many of her clients were stunned by the ruling.

"I cannot express how bad I feel for the couples that lost their embryos, so my initial knee-jerk reaction is my goodness, I feel so bad for those families," she said. 

"However, when I read the Supreme Court opinion, and I started seeing that they were calling an embryo a child, I was terrified.

"Because now Alabama has utilised a very emotionally charged case as a vehicle to regulate women's reproductive choices to the date of conception." 

Patients forced to travel interstate as IVF treatments are suspended

The impact of the court's ruling was felt within days in Alabama, where at least three of the state's seven fertility clinics have paused IVF services because of legal risks to their teams.  

It's left patients like Gabby Goidel scrambling to make new arrangements interstate. 

"I don't think I've ever been this stressed in my life, this worried," she told CNN. 

Ms Goidel is pursuing IVF after experiencing several miscarriages. She started the daily injections needed ahead of an egg retrieval on the same day the court's decision was handed down.  

While her clinic told her she could still have her eggs retrieved in Alabama, she's worried about the unknowns and is instead travelling to Texas to continue her treatment there. 

"It just feels like every time I try [to have children], the rug gets pulled out from under me and my husband," she said. 

In the court's only dissenting opinion, Justice Greg Cook warned the ruling would "almost certainly" end the creation of frozen embryos in Alabama, and he urged state legislators to consider the matter.

Republican state senator Tim Melson has responded with a bill stating that while an in vitro embryo is considered "potential life", it's not an "unborn life" unless it's implanted into a uterus. 

State Democrats have also proposed legislation excluding an embryo from the Wrongful Death of a Minor Act.

But it's not yet clear how either effort could progress.  

Another reproductive rights fight in a post-Roe America 

The IVF ruling is one of the most significant court decisions on reproductive issues in the US since the Supreme Court in Washington overturned Roe v Wade in 2022.  

That case ended the constitutional right to an abortion, allowing states such as Alabama to outlaw the procedure.  

The legal impact of the IVF decision is limited to Alabama, and law professor Mary Ziegler from the University of California, Davis said it was unlikely to be appealed to a higher court, given it dealt with a question of state law. 

But she believes similar decisions could follow in other states that also back the concept of "foetal personhood" — the extension of rights to a foetus.  

"Pro-life groups have already been pretty open about the fact that they're planning on taking advantage of this," she said.

"The anti-abortion movement is a foetal personhood movement. It's not just an anti-abortion movement."

Several prominent anti-abortion campaigners welcomed the Alabama decision, with the organisation Alliance Defending Freedom describing the ruling as a "tremendous victory for life".

Lila Rose, the founder of anti-abortion group Live Action, told her Instagram followers she hoped the decision was followed around the country.  

"They use the word 'frozen' embryo and it makes it sound kind of dehumanising honestly, but the reality is these are little babies, just a few weeks old," she said.

"And yes they’re frozen, which is one of the reasons IVF is so morally fraught because you’re literally freezing a new human life.” 

Australia drawn into argument over America's 'Wild West' IVF sector  

About 2 per cent of babies born in the US each year are conceived using assisted reproductive technology, and IVF is the main form.   

But Chief Justice Tom Parker argued the sector had long been under-regulated across the country, "with some commentators even comparing it to the Wild West":

"I note briefly that many other Westernised countries have adopted IVF practices or regulations that allow IVF to continue while drastically reducing the chances of embryos being killed."

Justice Parker referenced a code of practice from the Fertility Society of Australia and New Zealand (FSANZ) to suggest "physicians usually make only one embryo at a time" in those countries.

However, the FSANZ said that was inaccurate.

FSANZ president Petra Wale said IVF in Australia often involved creating multiple embryos, with the most viable embryo selected for transfer. If that initial attempt at pregnancy fails, the remaining embryos can be used in subsequent transfers without requiring the patient to undergo another round of stimulation and egg retrieval.

"The misrepresentation of our code as focusing on the creation of embryos, rather than the transfer process, overlooks the complexities of fertility treatments and the considerations that inform clinical decisions," Dr Wale told the ABC.

Fertility specialist Alex Polyakov, an associate professor at the University of Melbourne, said the Alabama judgement "borders on insanity".

"We have to remember that on average … every embryo gives you at most 35 to 40 per cent chance of falling pregnant," he said. 

"And so the majority of people who have an embryo transfer don't, in fact, get pregnant.

"If we have frozen embryos stored, we can use them and it's a much simpler process than going through another egg collection."

A political fight intensifies in an election year

Reproductive rights were already in focus ahead of this year's presidential race in the wake of Roe v Wade falling, and President Joe Biden has publicly tied that decision to the one in Alabama.

"The disregard for women's ability to make these decisions for themselves and their families is outrageous and unacceptable," he said in a statement. 

"Make no mistake: this is a direct result of the overturning of Roe v. Wade."

But former president Donald Trump, whose appointment of three justices to the Supreme Court facilitated the end of Roe, has criticised the Alabama decision. He's used social media to call on the Alabama legislature to "find an immediate solution":

"Like the OVERWHELMING MAJORITY of Americans, including the VAST MAJORITY of Republicans, Conservatives, Christians, and Pro-Life Americans, I strongly support the availability of IVF for couples who are trying to have a precious baby."

His rival for the Republican nomination, Nikki Haley, says she personally considers embryos to be babies but believes Alabama legislators should "go back and look at the law". 

In the meantime, Alabama lawyer AshLeigh Meyer Dunham is trying to help her clients navigate the uncertainty before them.  

"If this happened at the time that I was looking at possibly going through IVF, I don't know if I could have gone through with it," she said. 

"I probably wouldn't have the wonderful daughter that I have now."
