How to Write a Visit Report

Whether you’re a student or a professional, a visit report helps you document the procedures and processes at an industrial or corporate location. These reports are fairly straightforward. Describe the site first and explain what you did while you were there. If required, reflect on what you learned during your visit. No additional research or information is needed.

Things You Should Know

  • Check the report requirements before writing down general information about the visit. Explain the site’s purpose, operations, and what happened during the visit.
  • Identify the site’s strengths and weaknesses. Add recommendations for improvement and describe what you learned there if you’re a student.
  • Give your report a title page, write clearly and objectively, and include relevant pictures in the report. Proofread it carefully before submitting it.

Describing the Site

Step 1 Look over the requirements of your visit report.

  • Reports are usually only 2-3 pages long, but in some cases, these reports may be much longer.
  • In some cases, you may be asked to give recommendations or opinions about the site. In other cases, you will be asked only to describe the site.
  • Ask your boss or instructor for models of other visit reports. If you can't get a model, look up samples online.

Step 2 Start the paper with general information about the visit.

  • If you visited a factory, explain what it is producing and what equipment it uses.
  • If you visited a construction site, describe what is being constructed and how far along the construction is. You should also describe the terrain of the site and the layout.
  • If you’re visiting a business, describe what the business does. State which department or part of the business you visited.
  • If you’re visiting a school, identify which grades they teach. Note how many students attend the school. Name the teachers whose classes you observed.

Step 4 Explain what happened during the visit in chronological order.

  • Who did you talk to? What did they tell you?
  • What did you see at the site?
  • What events took place? Did you attend a seminar, Q&A session, or interview?
  • Did you see any demonstrations of equipment or techniques?

Step 5 Summarize the operations at the site.

  • For example, at a car factory, describe whether the cars are made by robots or humans. Describe each step of the assembly line.
  • If you're visiting a business, talk about different departments within the business. Describe their corporate structure and identify what programs they use to conduct their business.

Reflecting on Your Visit

Step 1 Describe what you learned at the site if you’re a student.

  • Is there something you didn’t realize before that you learned while at the site?
  • Who at the site provided helpful information?
  • What was your favorite part of the visit and why?

Step 2 Identify the strengths and weaknesses of the site.

  • For example, you might state that the factory uses the latest technology but point out that employees need more training to work with the new equipment.
  • If there was anything important left out of the visit, state what it was. For example, maybe you were hoping to see the main factory floor or to talk to the manager.

Step 3 Provide recommendations for improvement if required.

  • Tailor your recommendations to the organization or institution that owns the site. What is practical and reasonable for them to do to improve their site?
  • Be specific. Don’t just say they need to improve infrastructure. State what type of equipment they need or give advice on how to improve employee morale.

Formatting Your Report

Step 1 Add a title page to the beginning of your report.

  • If you are following a certain style guideline, like APA or Chicago style, make sure to format the title page according to the rules of the handbook.

Step 2 Write in clear and objective language.

  • Don’t just say “the visit was interesting” or “I was bored.” Be specific when describing what you learned or saw.

Step 3 Include any relevant pictures if desired.

Site Visit Analysis and Report: How to conduct and evaluate your first architecture site visit

Here we will cover everything you need to know about of how to approach your first site visit analysis for a new project, what to do when physically there, and how to eventuate and summarize the information you collect.

However before visiting for the first time we highly recommend that you carry out desktop study beforehand, as this will provide an important initial understanding of the site and generate far better results and more refined questions once there.

Conducting an architecture site visit analysis

A site visit analysis is a comprehensive report that summarizes the findings of a physical inspection of a potential development site. It includes information on the site’s physical characteristics, location, surrounding area, demographic information, environmental impact, zoning regulations, traffic flow, and recommendations for development.

The report synthesizes all gathered information to provide a comprehensive understanding of the site and its potential.

What to look for?

Once there, there are a whole number of important areas and items that need to be studied and recorded, some of which would have already been identified during your desktop study, but as a starting point we’ve produced the below list of all the key areas:

We suggest that you take these with you and tick them off as they are found, so not to miss anything.

  • Entrance and access points (both pedestrian and vehicle)
  • Security (gates, surveillance)
  • Travelling to the site (road types and suitability, safety, public transport)
  • Boundary treatment (fencing, vegetation, land form, water)
  • Extent of boundary (does it match the survey/OS map)
  • Circulation (existing travel routes within the site)
  • Noise levels (quiet and loud areas)
  • Services (electric, gas, water, sewage)
  • Existing buildings (condition? Relevant? Protected?)
  • Existing landscape features (condition? Relevant? Protected?)
  • Neighbouring buildings (local vernacular, protected?)
  • Views in and out of the site (areas to screen off and areas to draw attention to)
  • Tree’s and vegetation (protected and rare species)
  • Ecology (any areas likely to be home to protected species)
  • Orientation (sun and wind paths)
  • Light levels (areas in direct sunlight, shaded areas, dappled light)
  • Accessibility (disability access)
  • Surrounding context (historical, heritage, conservation area, SSSI, AONB)
  • Existing materials in and around the site
  • Topography (site levels)
  • Flood level (is it likely to flood)
  • Soil and ground conditions (types and suitability)
  • Existing legal agreements (where are the rights of way, covenants)
  • Hazards (Electricity lines, Drainage, Telephone lines, Sub-stations)

Where to start

You want to begin documenting your visit as soon as you arrive, as the approach and entrance to your site are just as important as the site itself. If you’re desktop study didn’t highlight the possible routes and methods of transport to and from the site, then this needs to be recorded also.

Documenting your first impressions is vitally important, ask yourself; what do you see as you enter the site? what do you hear? what do you feel? (…what senses are the first to be triggered), you will only get one chance to do this properly and so you need to make it count!

…and don’t forget to include the location of the elements you record, when noting it down on your site map or survey. By the end of your visit, you should barley be able to read whats under all your notes …write down everything!

Moving on from first impressions, you should plan to walk around the site as least twice (as a minimum) to ensure that nothing is missed, so leave enough time to make a least two loops, noting down and photographing everything that you feel is relevant, no matter how small.

…there’s nothing worse than getting back to the studio and realizing you forgot to document something.

We like to use the check list supplied above and:

  • Firstly walk around the site whilst annotating a site plan
  • Secondly with a camera …photographing everything
  • and thirdly with both …just in case something has been missed

This way we can focus on one task at a time, helping to ensure we gather everything we need.

In terms of a camera, and depending on your budget we suggest looking one these three options (but a phone is just as good):

  • Sony DSCW800 Digital Compact Camera
  • Sony DSCWX350 Digital Compact Camera
  • Canon EOS 1300D DSLR Camera

It can be difficult to identify certain elements, and some may only be noticeable from a professional survey, such as underground services and precise spot levels. But approximations of such locations and heights are a good start and can serve as a reminder for further investigation.

If accessible you can of course take your own measurements and so this is where a tape measure and/or distance meter will come in handy.

What to take with you

Firstly look at the weather, you wont have a good time if your not dressed appropriately, and this applies to protecting your notes and equipment as well as yourself.

…a simple quick check, can make or break a visit, arranging to go on sunny day will also give you the best site photographs, which could also be used in future CGI’s and presentation material. 

If the site is derelict, or has potentially dangerous or hazardous elements, it is likely that you will require personal protection equipment (otherwise known as PPE) so make sure this is organised before setting off.

As a minimum you want to take with you a camera, a pen and an OS map. Google Maps can provide a temporary (though very basic) version, but a much preferred scaled version that can normally be obtained through your university or practice via such companies as:

  • Digimap – digimap.edina.ac.uk
  • Xero CAD – xerocad.co.uk
  • CAD Mapper – cadm a pper.com (free account available)

As mentioned, you will want to make notes, and record everything you observe, experience and hear all over this map. So print out a couple of copies at a usable and convenient size.

A camera is essential in documenting the site, and the pictures taken during your visit are likely to be used on a daily basis throughout your project. So once again make sure you document and record everything.

Pictures should be taken from all distances, close zoomed-in sections of materials and textures along with shots of the site from a distance to include the area as a whole and within its context.

Note pads are important for obvious reasons, we prefer an A5 sized pad, as this is much easier to carry and hold than an A4 one.

Tape measures can be useful, but we never go on a site visit without a distance meter.

…and lastly if you’re visiting on your own, don’t forget to tell someone where you’ll be and take your phone with a charged battery.

  • Weather check
  • Print out our “what to look for” checklist
  • Site map (at least 2 copies)
  • PPE equipment
  • Scale ruler

How to Write a Visit Report

One of the most reliable ways to determine how well a business is faring is by using a visit report. Are your external operations doing as well as they should? Is your preschool or care facility up to code and ready for licensing? By writing a comprehensive visit report, you can determine whether a wide variety of business objectives are being met. Visit reports, sometimes called trip reports, are a great way to find out whether your business standards are being carried out on site in the way you planned.

Format the Report

Visit reports are business documents. Depending on the organization or industry, the report may follow a memorandum format or a business template. Generally, choose a memo format if the visit report is going to a group of internal leadership members. Choose a more formal business report template if the visit report will be provided to external sources. Use standard business formatting that includes professional type fonts such as Times New Roman or Arial. Maintain 1-inch margins.

State the Objectives

Explain the reason for the visit. The objectives include the frequency of the visit, along with primary metrics or areas of review. Provide Future Action Plans 

State when future visits are scheduled if any, and whether these are predetermined or a result of the recent visit. For example, this may have been the third annual visit on a quarterly schedule. Provide recommendations for improvements. If certain action plans are defined, state these in detail. This provides a success metric for the next visit.

The Art and Science of Site Monitoring Visit Reports

An experienced CRA may follow all the rules and requirements for writing a site monitoring visit report (SMVR), but there are nuances that can make the difference between a report that simply follows that formula and one that really paints a picture its audience can understand.

Roslyn Hennessey, senior project manager at Westat describes the ideal SMVR as “a snapshot in time of what’s happening at the site.” To develop that snapshot, start by asking who will read the report, she advises. There are several types of audiences for SMVRs, all of which have slightly different interests:

  • The sponsor wants to know about the performance of the site — is it in compliance with the protocol, are improvements needed, are there any major concerns;
  • The PI and site staff want to know if additional training or procedural changes are needed;
  • Regulatory authorities and auditors want to know if the site is in compliance and, if not, what corrective action was taken;
  • Future monitors need enough information to ensure a smooth hand-off from one CRA to another.

It’s important to give all of the key stakeholders the “heart or the core of the information that they are most interested in,” she says.

And although an SMVR should be comprehensive, it also needs to be concise to avoid burying important information or tiring the reader. Writing an SMVR is an exercise in deciding what information needs to go in and what needs to stay out.

“We really need to get more efficient in terms of going just from point A to B,” she says, “but in that A to B, we need to include some important things.”

Hennessey recommends starting at the end. A monitor usually will have an overall message to convey in the report and should choose the information that supports that message. “Know what you want to say ahead of time and write to your conclusion,” she says. “What do I need to say to get me to that conclusion?”

Be analytical in what you write, she advises. A good SMVR is more than an “information dump.” When writing the report, provide the reader with some context instead of just transcribing notes. Explain what those notes indicate, Hennessey urges. Look for common denominators in the findings and identify root causes of problems. Pay attention to trends, from a single visit and across several visits.

In addition to being analytical, an SMVR should be objective. Elizabeth Weeks-Rowe, a clinical research training consultant, advises SMVR authors to keep the human factor in mind. A monitor may encounter many different personalities in a trial, but they should not be reflected in the site report.

“Taking the emotion out of verbiage is critical,” says Weeks-Rowe. “Be factual, not emotional.” If, for example, a principal investigator is less than professional, this impression should not be included in the report. Instead, address any outcome of that behavior. Making sure that all findings are actionable helps avoid an emotional narrative, she says.

Hennessey notes that an attentive monitor can enhance communication between the site and the sponsor. “Sometimes the investigators feel like the sponsor is too far removed,” she says, and wonder “where is my voice heard?” The investigator may feel more comfortable communicating with the CRA, who then passes the information on to the sponsor via the SMVR.

The last step in writing an effective SMVR is recommending corrective actions, Hennessey says. All issues/findings noted in the report should have an associated action and/or resolution. Have a “no finding left behind” policy, she advises. But, she cautions, don’t be overly prescriptive. It’s important that the PI and site staff take responsibility for problems and solutions.

Both Hennessey and Weeks-Rowe stress taking care of the basics, carefully reviewing the SMVR for misspellings and grammatical errors. Flaws in the presentation, Hennessey says, distract from the content and message and ultimately hurt the credibility of the monitor and the company.

Ultimately, the SMVR must be able to stand the test of time, Hennessey says. Trials can last for years and go through several CRAs along the way. “It’s not often that a CRA gets to work with a site from initiation to closeout,” she notes. And a regulatory request for further information or an audit can come at any time. In one case, she says, the FDA came back to question the SMVR 10 years after the fact.

How the Key Bridge Collapsed in Baltimore: Maps and Photos

On Tuesday, a major bridge in Baltimore collapsed into the water seconds after it was struck by a cargo ship, sending vehicles on the bridge into the river below. The cause of the strike has yet to be determined.

The ship, a 948-foot-long cargo vessel called Dali, was about a half hour into its journey toward Colombo, Sri Lanka, when it hit a main pillar of the bridge. All crew members are safe, according to the ship owners.

A mayday call from the ship gave officials enough time to stop traffic at both ends of the bridge. The waters where the bridge collapsed are about 50 feet deep. By Tuesday morning, six construction workers who had been fixing potholes on the bridge remained missing as divers and other emergency workers on boats and helicopters continued to search for them. Two others had been rescued, and one was in the hospital.

The Francis Scott Key Bridge was built in 1977 and carried more than 12.4 million vehicles last year. The bridge was one of the three major ways to cross the Patapsco River and formed part of Baltimore’s beltway.

Gov. Wes Moore declared a state of emergency for Maryland and said that his office was in close communication with Pete Buttigieg, the U.S. transportation secretary. The White House issued a statement saying that President Biden had been briefed on the collapse.

Coleman, an Opelousas, Louisiana native, started his college career at Michigan State after shining at Opelousas Catholic. In two seasons with the Spartans, he had 65 catches (58 in 2022) for 848 yards (798 in 2022) and eight touchdowns (seven in 2022). He also appeared in six games for Tom Izzo’s Michigan State basketball team.

However, he transferred to Florida State for the 2023 season where he shined with Jordan Travis at quarterback. The shifty receiver caught 50 passes for 658 yards and 11 touchdowns in an offense where he was the second option.

At the NFL Scouting Combine in Indianapolis, Coleman ran a 4.61 40 along with a 38” vertical jump. He’s currently being compared to Atlanta Falcons star receiver Drake London, a 1st round pick two years ago.

Coleman may not make it through the first round, so it’s possible that the Black and Gold trade back into the first round to grab him if they’re interested enough, but we’ll have to keep an eye on the situation as it plays out over the next few weeks.

Construction site observation report: What's required (and a sample)

What is a site observation report used for in construction.

A site observation report is the report which documents the general and detailed observations a worker or inspector makes on site - on a particular day and at a particular time.

The idea behind conducting observations and creating observation reports is to step back and make a conscious and objective assessment of project progress, as well as general site activity, safety or environmental findings and evidence of anything out of the ordinary.

The 'reporting' part of the report is obviously the first step, with the point of reporting being that the findings will be made actionable, either by the observing party or other workers and teams.

A sample construction site observation report

The observing part of the observation is done by a human; either an inspector or select employee, while the reporting part is conduced with a site observation report template - which can be created with efficient commercial management software, word, excel or other paper and doc builders.

The site observation report template should serve as the observation framework, and should guide but not constrain the observers movements and actions.

Depending on the level of detail and purpose of the observation, the person conducting the report may document more or less findings. If your daily reporting processes and records are strong, the inspector may be looking for outlying details. If you daily reporting is hit and miss or non-existent, then the report should cover more of the general progress as well as those outlying details.

As you can see in the sample report below, there are a few requisite details which need to be on every report, as well as some template sections for unique findings. The necessary reporting fields include:

  • The observation date and time
  • The project or job being inspeted
  • The conditions (weather etc.)
  • Inspector signoff once the report is completed

Outside of these details, the report will cover any new or observed findings:

  • Party, description of work completed and supporting photos
  • Checkbox safety questions which will focus on the unique elements of your site and safety efforts
  • Discussion notes from site which provide more context around what was found as well as what was already partially covered and known about on site

Obviously the more detail provided in the site activity table the better, with detailed descriptions and detailed photos providing a heap of value for other parties who did not do the walkaround, as well as context for looking back at the records later on down the road.

Why are site observation reports important.

The site report serves as an important project document which helps companies and workers understand:

  • What were the activities which took place on day 'X', and were these activities expected and in accordance with good work practices
  • How things are going overall (are we on track, behind schedule etc.)
  • In the day-to-day execution of work, have we been sloppy with safety or environmental actions
  • Is there anything we have missed?

It also serves to help inform multiple stakeholders of progress. A site observation report may be shared with subcontractors and contractors scheduled for work to help them understand what's happening, and will certainly be shared internally to keep functions and teams on the same page.

While site diaries and daily progress report forms help keep commercial and production teams in the loop on progress, deliveries and other work related items, they don't typically look at the project holistically (from a birds eye view).

It's important to do both the daily reporting at job and execution level, as well as to maintain a solid and regular cadence of observations.

Together, these forms and practices create an accurate view of what's happening and help companies and people mitigate mistakes and make more informed decisions - which results in better outcomes overall.

The quality of your observation efforts and reports are also extremely important. These reports are the reports which which are sent to important project parties, and they should be created, completed and formatted to represent this fact.

Poorly structured and fuzzy reports create misunderstanding, unnecessary back and forth and can create wasted time and resentment between parties.

Projects get delayed and end up over budget because people don't have the information they need to make informed decisions. Good observation reports help fill this void with useful and timely information of what's actually happening on the ground.

Maryland bridge collapse: 6 unaccounted for after cargo ship hits Francis Scott Key Bridge

BALTIMORE — A major bridge collapsed in Baltimore early Tuesday morning after it was hit by a container ship, sending several vehicles plunging into the dark water below, sparking an intense search for survivors and shutting down one of the nation's busiest ports.

Maryland Gov. Wes Moore declared a state of emergency and said the calamity that knocked down the Francis Scott Key Bridge was likely the result of an accident and not an act of terrorism.

"Our state is in shock," Moore said.

Moore said the cargo ship Dali notified authorities of a "power issue" and issued a mayday moments before the 984-foot vessel slammed into a bridge support at a speed of eight knots, which is about nine miles per hour.

Meanwhile, rescue crews using sonar detected at least five vehicles in the frigid 50-foot-deep water, including three passengers cars, a cement truck and another vehicle of some kind, Moore said.

An unknown number of workers were doing repairs on the bridge when the ship hit a support pillar and at least six people were still believed to be missing, Moore said.

"Our exclusive focus is on saving lives," the governor said.

An 'active search-and-rescue posture'

Earlier, two people were rescued from the water, Baltimore Fire Chief James Wallace said.

One was in good condition and refused treatment, Wallace said. The other was seriously injured and was being treated in a trauma center.

“We are still very much in an active search-and-rescue posture at this point,” Wallace said.

Moore said the bridge, which is about a mile-and-a-half long and carries Interstate 695 over the Patapsco River southeast of Baltimore, was "fully up to code."

Transportation Secretary Pete Buttigieg has been monitoring the situation and a team of National Transportation Safety Board investigators was already at the bridge.

President Joe Biden was expected to weigh-in on the ongoing situation later Tuesday before he departs for North Carolina.

“Our hearts go out to the families of those who remain missing as a result of this horrific incident,” a senior White House official said earlier.

Dramatic video captured the moment a little after 1 a.m. Tuesday when the Dali struck a support and sent the bridge tumbling into the water. A livestream showed cars and trucks on the bridge just before the collision. The boat did not sink and its lights remained on.

At another news conference, Maryland Department of Transportation Secretary Paul Wiedefeld said the workers on the bridge were repairing concrete ducts when the ship crashed into the structure.

'My house shook'

The U.S. Coast Guard told NBC News that it received a report at 1:27 a.m. ET that a “motor vessel made impact with the bridge” and confirmed it was the Dali, a container ship sailing under a Singaporean flag that was heading for Sri Lanka.

Bobby Haines, who lives in Dundalk in Baltimore County, said he felt the impact of the bridge collapse from his house nearby.

"I woke up at 1:30 this morning and my house shook and I was freaking out," he said. "I thought it was an earthquake and to find out it was a bridge is really, really scary."

At the time of the crash, contractors were doing concrete repair on the bridge, Wiedefeld told reporters.

A representative from Brawner Builders Inc. said that the company had a construction crew on the bridge when it collapsed. 

The representative declined to provide additional details, including how many employees were working at the time or are still missing.

The bridge collapsed after being struck by a container ship, sending multiple vehicles and up to 20 people plunging into the harbor below.

Shipping giant Maersk confirmed in a statement that the Dali, operated and managed by a company called Synergy Group, was charted to transport its customers’ cargo.

Synergy said in a statement that it had "collided with one of the pillars of the Francis Scott Key Bridge, Baltimore whilst under pilotage with two pilots onboard."

The company said all 22 crew members onboard at the time of the crash were accounted for and there were no injuries or any oil pollution.

Grace Ocean Private Ltd., the Singapore-based registered owner of the ship, did not immediately respond to a request for comment from NBC News.

Families of bridge workers wait for updates

Baltimore Mayor Brandon M. Scott said at a news conference: "We have to first and foremost pray for all of those who are impacted, those families, and pray for our first responders and thank them, all of them, [for] working together — city, state, local — to make sure that we are working through this tragedy."

A group of people who said they were family members of workers employed by Brawner Builders Inc. gathered at a Royal Farms convenience store near the entrance of the bridge.

They said their loved ones were working on the bridge at the time. One woman told NBC News that her father-in-law, Miguel Luna, was among the workers.

They went to the store because it was the closest they could get to the bridge, but were yet to get any formal guidance as to the status of their loved ones, they said.

'A long road in front of us'

Built in 1977 and referred to locally as the Key Bridge, the structure was later named after the author of the American national anthem.

The bridge is more than 8,500 feet, or 1.2 miles, long in total. Its main section spans 1,200 feet and was one of the longest continuous truss bridges in the world upon its completion, according to the National Steel Bridge Alliance .

About 31,000 vehicles a day use the bridge, which equals 11.3 million vehicles per year, according to the Maryland Transportation Authority.

The river and the Port of Baltimore are both key to the shipping industry on the East Coast, generating more than $3.3 billion a year and directly employing more than 15,000 people.

Asked what people in Baltimore can expect going forward, the state's transportation secretary said it is too early to tell.

"Obviously we reached out to a number of engineering companies so obviously we have a long road in front of us," Wiedefeld said.
